Description: Under the supervision of a R.N. or L.P.N., provides quality patient care in a safe and efficient manner.

Certification by State of Illinois
EXPERIENCE: None required
SPECIAL QUALIFICATIONS:
None

Duties:
Consistently assess and identify patient care needs by gathering patient information through objective and subjective data.
Consistently provide care by completion of patients’ ADL’s and by other additional aspects of patient care.
Communicate effectively with customers by providing information for patients related to care.
Accurately documents all nursing care provided and adheres to nursing standards.
Maintain a safe work environment by adhering to the policies and procedures of the department, the organization, OSHA and the Joint Commission.
Round on all assigned patients using POINT according to unit policy.
Regularly receives, sorts, and delivers supplies.
Consistently maintain a clean safe work environment by ensuring appropriate operation on equipment.
Plan for patient care by prioritizing needs of the patients and supporting the plan of care through assigned duties on a regular basis.
Communicate effectively with RN and/or LPN related to the care of patients and changes in the condition of patients.
