The primary purpose of the QIDP is to provide case management services to the assigned Community Learning Center (CLC) individuals. The QIDP is responsible for developing and overseeing the implementation of the Individual Support Plan for 25-30 individuals served. They are directly responsible for facilitating the achievement of personal outcome and for overseeing the delivery of services across settings in order to enhance the well being, quality of life, opportunities and achievement of personal outcomes for individuals served on their caseload. The QIDP is expected to uphold the Mission & Values of the Association.
Description of Responsibilities:
The QIDP is responsible for developing and overseeing the implementation of the Individual Support Plan for individuals served in our Community Learning Center (CLC).
This position works collaboratively across agency programs and with external service providers to promote the delivery of quality services to people served.
The QIDP develops and implements person-centered plans that facilitate the achievement of personal outcome measures.
This position is directly responsible for the oversight of services and supports in our Communtiy Learing Centers. The QIDP is expected to uphold the Mission & Values of the Association.
