Job Description

1. Position Identification:
a. Position Title: QIDP
b. Job Location: DT or Assigned Cottage(s)
2. Position Relationships:
a. Reports to: QIDP Supervisor
b. Positions Directly Supervised: None
3. The Resident is at the heart of all we do at Mt. St. Joseph (MSJ). The primary responsibility of ALL staff is to our residents. We must ensure that their care, needs, and safety are being met at all times.
4. General Purpose of Position:
a. Responsible for the planning, development, organization, implementation and evaluation of case management activities/ individual program plans (IPP) for assigned caseload
5. Essential Position Functions:
a. Plan, develop, implement, supervise and evaluate IPP and overall plan of care for assigned residents. Ensure validity and appropriateness of care and programs received/ attended by residents
b. Advocate for residents – work with housemothers to ensure residents’ needs are met and rights upheld
c. Coordinate annual assessments for caseload, determining strengths and needs with inter-disciplinary team (IDT) and inform all appropriate departments/ staff of results
d. Complete required paperwork on a timely basis
e. Coordinate acquisition of appropriate training material and supplies necessary for program implementation
f. Coordinate as needed training for implementation of established programs
g. Monitor progress and implementation of programs in residents’ cottages and vocational programs (which includes on-site observations)
h. Collect and review data on a minimum of a monthly basis to assure accurate reporting. Intervene when necessary to assure proper documentation and implementation
i. Document QIDP monthly progress notes, adjusting programs as needed
j. Coordinate monthly meetings with housemother and appropriate staff (hab aides, pertinent ancillary staff, etc.)
k. Assist housemother with ensuring adequate supervision
l. Intervene during resident maladaptive behavior episodes when available
m. Follow up, as necessary, in coordination with housemother/house manager regarding all resident concerns and needs
n. Coordinate with Behavior Specialist on Behavior Programs
o. Other duties as assigned by supervisor.
6. Secondary Position Functions:
a. Monitor outside vocational jobs
b. Participate in committees, as assigned
c. Assist in cottage when necessary
d. Help housemothers plan and implement resident community outings on a regular basis

7. Critical Knowledge, Skills, and Expertise Needed Specific to this Position:
a. BA/ BS in a Human Relations field
b. One (1) year experience with MR/ DD population
c. Must maintain current CPR and First Aid certification and demonstrate ability to respond appropriately in an emergency
d. Basic computer knowledge
e. A valid driver’s license
8. Physical Demands and Working Conditions:
a. Must be able to speak, hear, and see (with or without reasonable accommodations) to conduct the essential job functions for this position
b. Must be able to get around within the facility safely (with or without reasonable accommodations) in order to complete the essential job functions for this position
c. 40 hour work week; flexibility needed including some early mornings, late afternoons and weekend hours on occasion
d. May include lifting and transporting residents
e. May include dealing with maladaptive behaviors
9. Equipment Used in Performing Functions:
a. Computer, scanner, copier, fax, DVD player/ projector (especially used in trainings)
b. Facility vehicles
