The Registered Nurse, in OSF Home Care is responsible for a scope of care that encompasses all ages from infancy through geriatric. The RN demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. This requires that the individual demonstrate knowledge of the principles of growth and development as well as the physical, emotional and psychosocial needs of the patient population served. Coordinates inter-disciplinary care to patients, which may include nursing, social work, home care aide, bereavement, volunteer, chaplain, therapy, home medical equipment and home infusion services as needed. Utilizes physiological, psychological and spiritual knowledge to care for a variety of patients and their caregivers and recognizes the direct relationship between cost effective and efficient provision of care and services. Provides patient-centric care, utilizing critical thinking skills to ensure positive patient outcomes. Allows flexibility in their schedule to accommodate patient needs.
Currently licensed as a Registered Nurse in the state(s) in which they are practicing. BSN or BSN within 6 years of hire and enrolled within 18 months (signed agreement required)
Provides own transportation and auto insurance and abides by all laws, rules and recommendations for safe driving while on duty.
Maintains a valid driver’s license.
Demonstrates ability to work independently and execute own workload utilizing IC2it accountability tools.
Must be able to operate in a virtual environment.
Must be able to use and operate medical equipment.
Demonstrates critical thinking skills.
Expected to take night and weekend calls on a rotating basis, and make home visits to patients during the evening, night, weekend and/or holiday hours as required.
Time Management skills necessary in order to manage assigned caseload.
One-year recent nursing experience preferred
Hospice and/or home health experience preferred
