Job Description

We are looking to add an LPN to render professional nursing care for the comfort and well-being of the members. Provide for the bio psycho-social needs of the members. Administer prescribed medication, maintains residents’ medical records and may assist physician during treatment and observation.
Essential Functions & Responsibilities
Support practices that foster compliance to LHSA medication administration Policies and Procedure.
Ensure that electronic client records are kept up to date.
Ensure center is fully staffed to meet the objectives of the Lutheran Homes and State and Federal regulations.
Responsible for all training of staff to meet state regulations.
Demonstrate proactive approaches to reducing customer concerns and liability.
Work through grievances to ensure a satisfactory resolution for all parties.
Administer medications and conduct a conscientious and accurate medication pass, maintain the medical record. Follows order for treatments and assures appropriate care is being rendered. Makes corrections as necessary by visiting each member and contacts physician if necessary.
Monitor member’s nutritional services.
Evaluate and assess medical, social, psychological, spiritual, restorative and rehabilitative needs and goals of members.
Promote a holistic environment.
Review and write progress notes, as well as review treatment plans and resident care plans.
Notify physician and next of kin when warranted.
Obtain and transcribe physician’s orders.
Communicate with families regarding treatment plans.
As appropriate, provide information to members’ relatives and visitors.
Provide leadership and guidance to all adult day staff and promote team work through problem solving issues related to both member care and staff concerns.
Provide performance feedback to staff as appropriate.
Support and monitor staff performance of meal service and housekeeping roles.
Contact maintenance and/or housekeeping department for services needed to guarantee a safe environment for customers and staff.
