Overview

Position Summary

Provides assistance to patients and their families in the form of discharge planning, counseling and resource referral appropriate to the ages of the patients served. Responsible for consultation/supervision of the provision of social work services including, but not limited to, recommendations for changes in service delivery. Referrals have documented care plans defined by policy. Attends and participates in scheduled client care conferences, participates in projects as assigned. Charts have documented education/interventions re: assessment of client’s/family’s needs for information and coordination of referrals to appropriate community agencies/resources.

Education

Bachelors-Social Work-Required

Experience

Prior experience in social work or past social service clinical experience directly transferrable to a healthcare environment preferred.

Certifications, Licenses and Registrations

Licensed in the state of practice-Required
