Job Summary
Works in collaboration with Physicians, Administrator, Nurse Managers and other health care providers to coordinate the care of patients. Provides professional medical care for patients following established standards and practices.

Certificates, Licenses, Registrations
Maintains necessary certification, licensure, and hospital privileges. CPR certification is also required.
Education and/or Experience
Graduate of an accredited school.

PRIMARY JOB DUTIES

Performs complete and thorough assessments in assigned clinical duties.
Documents all encounters in a timely fashion according to the documentation policy (Documents all care given, phone contacts, and patient response to care).
Provides patient education and enlists patient support of the treatment plan.
Continually invests in relationship with the collaborating physician(s) and keeps CP apprised of patient care issues.
Discusses and communicates with other department team members and specialists as needed in providing medical care to patients and their families.
Provides support to nurses and support staff in handling patient phone calls; talks with patients directly as needed; helps achieve departmental goals and attends department meetings.
Provides ancillary results to patients in a timely manner and documents the follow-up.
Invests in self by continually advancing clinical knowledge base.
Works in collaboration with the Physicians and other health care providers.
Assist patients in setting attainable goals and developing action plans.
Other duties as assigned.
