The Bariatric Team is instrumental in providing life-changing patient education, support, and care coordination for patients seeking surgical weight loss options. On this team, you are guaranteed to learn, grow, and feel welcome! The Dietitian provides nutrition education and counseling to clinic patients and patient groups. Assesses the nutritional needs of the outpatient, develops and implements a nutrition care plan. Develops and implements programs for the general public, community organizations and for professional staff. Qualifications: EDUCATIONAL REQUIREMENTS Bachelors Food & Nutrition or Bachelors Dietetics CERTIFICATION & LICENSURE REQUIREMENTS Registered Dietician – Commission on Dietetic Registration and Dietitian License Illinois OTHER REQUIREMENTS Possesses planning, leadership and organizational skills. Demonstrates ability to accept and institute change. Communicates effectively and works collaboratively with others. Essential Functions:+ Provide nutrition services and medical nutrition therapy integrated with the patient’s medical goals. Identify and provide age specific nutrition counseling to meet the cultural needs of the patients and their families
+ Evaluate, interpret, monitor and document the nutritional status and nutritional needs of patients using established standards of care and practice guidelines.
+ Develop, review, update and implement educational materials to meet the needs of patients and professionals
+ Pre-operative nutrition assessment and documentation for patients seeking weight loss surgery
+ Pre-operative weight management and nutritional education for patients seeking weight loss surgery
+ Provides education to patient regarding dietary guidelines for the immediate post-operative period after weight loss surgery
+ Provides ongoing patient education for diet advancement and continues weight management after weight loss surgery
+ Participation in the Bariatric multi-disciplinary committees to improve patient outcomes
+ Leads Bariatric Patient Support Group to provide ingoing education and support for patients on their weight loss journey in a group setting.
+ Leads the Bariatric Information Seminar to provide information regarding weight loss surgery in a group setting.
