Health Promoter – Lead Wellness Operation

Must be bilingual.

This position is not remote.

MA experience is preferred but not required.

Responsibilities

Lead all aspects of operations for health and wellness events.

+ Coordinate logistic requirements for events including scheduling, booking space, equipment, and educational materials.

+ Determine appropriate staff and contract service providers based on type of event and expected registration.

+ Communicate with clients to ensure their needs are met and events are successful.

+ Monitor the quality and efficiency of event operations and implement improvements as indicated.

+ Monitor and/or conduct education and health activities at events.

Requirements

Education:

+ High school diploma/GED with 2 years of experience, or Associate’s degree, or Technical degree required.

Work Experience:

+ 1 year of experience.

Additional Preferences

additional preferences - Performs telephonic outreach to UHH participants. Examples include but are not limited to: o Contacts patients who’ve been in the ER to make sure they have a follow-up plan and assists them in finding a provider if needed o Contacts Health Center patients with gaps in care, encouraging them to do what’s needed and assists them in scheduling care if needed (e.g. mammograms) o Contacts participants regarding vaccine education – Attends workplace onsite events to educate participants about benefits, promote the Health Center, distribute health education, locate providers, and encourage participants to seek care – Participates in Care Coordination huddles where the team reviews cases and develops follow-up plans for patients requiring further assistance – Assists at Health Center, including but not limited to: o Performs front desk work (answering phones, scheduling appointments) if MA is occupied or out of the office o Performs all MA work if trained and MA is out of the office and another replacement is not available o Collects and summarizes Patient Satisfaction at Health Center by texting the survey link to patients right after their appointment and compiling the results of the patient satisfaction surveys on a quarterly basis o Makes referrals to UHH programs, like the Diabetes program and Healthier Me (virtual diabetes self-management group workshops). – Additional duties as requested by the UHH Participant Engagement Supervisor to include, but not limited to, special projects, community outreach, home visits, etc. – Position is primarily based at the Health Center and may travel to local worksites in the Chicago area about once a week. It is possible, but not guaranteed, that there may be one day of work from home if desired depending on the type and amount of outreach work available.
