Job Description

BASIC FUNCTION: Under the direct supervision of the NOC Coordinator, supervises direct care staff, supervises and conducts implementation of each individual's Individual Service Plan, monitors programmatic documentation, supervises and participates in training direct care providers, supervises and participates in social and recreational activities, and maintains houses unit in clean, orderly fashion. DUTIES AND RESPONSIBILITIES: E.1 Monitors individuals served and staff whereabouts at all times and notes any variations and/or contacts appropriate supervisor when necessary. E.2 Reliable, prompt attendance, including shift change report. E.3 Distributes daily staff assignment/individual routine sheets, providing for equitable schedule rotation of job duties among assigned staff, and supervises staff in completion of assigned duties. E.4 Evaluation, with input and review by Resident Services Director/NOC Coordinator, of job performance of assigned direct care technicians, including any necessary disciplinary action. E 5 Actively participates as a member of the Interdisciplinary Team, providing input on program development of Individual Service Plans as directed. E.6 Monitors correct implementation of each individual's Individual Service Plan and discusses deficits and/or problem areas with same as they arise. JOB DESCRIPTION NOC TEAM LEADER Page 2 E.7 Completes programmatic documentation for all programs personally conducted and monitors completion of programmatic documentation by direct care staff on a daily basis. Ensure timely completion of any documentation deficiencies. E.8 Monitors complete and up-to-date charting for individual phone calls, visitors, menses, seizures, weights, and any necessary injury reports. E.9 Assists with meetings on a monthly basis, addressing questions and concerns and relaying pertinent information to Resident Services Director and QDIP. E.10 Supervises direct care staff and individuals in any emergency situation, following established guidelines/procedures for maximum safety and facility security. E.11 Contacts nursing staff regarding any individual health concerns promptly. 12 Assist NOC Coordinator/QDIP's in maintaining adequate supply of well-fitting clothing in good repair for all assigned individuals. E.13 Appropriately utilizes cleaning and sanitation supplies and monitors their use by direct care staff, ensuring restorative cleaning and repair requests are logged correctly. E.14 Maintains appropriate confidentiality regarding individuals, staff, and Agency issues. E.15 Monitor DSP's and check individuals routinely to ensure that check/change needs are being met in a timely manner. E.16 Ensures that positioning schedule is being followed. E.17 Must be able to safely use and maintain medical equipment reporting any issues to supervisor. E.18 Work in conjunction with nursing staff and Home Coordinator to ensure the medical needs of the individuals are being effectively communicated and met. E.19 Assist with providing monthly health related topics for discussions/training during staff meetings. E.20 Assist nursing staff as directed in obtaining individuals vital signs. E.21 Ensure that Milestone and CDC procedures and guidelines in regards to universal precautions, isolation procedures, and the use of PPE (personal protective equipment) are followed. E.22 In compliance with Milestone, Inc.'s philosophy and policies regarding Human Rights intervenes on behalf of the individuals in any questionable situation and reports any concerns or questions to the Supervisor-on-Duty immediately. E.23 Completes other duties as assigned as necessary to ensure the safe and effective functioning of the facility.
