Summary This incumbent will receive, review and adjudicate local prior authorization drug requests (PADRs) at the Captain James A. Lovell Federal Health Care Center. He/she will conduct education with providers at the point of ordering with emphasis on providing the most clinically appropriate, safe and cost-effective treatment to Veterans. In addition, the incumbent will evaluate and compare the differences between both the VA and DoD formularies. Responsibilities The Formulary Program Manager will receive, review and adjudicate local prior authorization drug requests (PADRs) at the Captain James A. Lovell Federal Health Care Center. He/she will conduct education with providers at the point of ordering with emphasis on providing the most clinically appropriate, safe and cost-effective treatment to patients. In addition, the incumbent will evaluate and compare the differences between both the VA and DoD formularies. He/she will also be responsible for implementing the VA and DoD formulary changes. The incumbent will work collaboratively with the local Pharmacy and Therapeutic Agents Review Committee, Pharmacoeconomics Clinical Pharmacy Specialist, Clinical Informatics and establish relationships with professional staff at FHCC to continually review and implement the safe, cost effective use of medications. The incumbent will also spend a portion of their time in an active clinical pharmacy practice as assigned. Communicates orally and in writing to persuade and influence clinical and management formulary decisions. 2. Maintains expert understanding of regulatory and quality standards for their program area. 3. Solves problems, coordinates and organizes responsibilities to maximize outcomes by formulary management. 4. Develops, upgrades, and disseminates acceptable criteria for the use of non-formulary medications and other high-cost medications and products to ensure cost-effective therapy without compromising the overall quality of patient care. Clinical: 1. Is directly involved in the design, implementation, and monitoring of therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas. 2. Actively involved in reviewing patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed. 3. Assumes patient care management functions as delineated in the Scope of Practice statement approved through the Federal Health Care Center (FHCC) Chief Medical Executive. 4. Monitors for and reports drug errors, adverse drug reactions, allergies, and patient compliance issues. Incumbent documents findings per facility procedures. Administrative: 1. Works with Administration and Staff to identify and implement new programs. 2. Develops and updates policies and procedures defining operational requirements of the section. 3. Serves as the Pharmacy Representative on various Medical Center committees (i.e.: P&T, Medication Management, etc.) 4. Coordinates quality assurance endeavors that relate to the use or outcomes of medication and clinical services. Works with Pharmacy Administration to design and implement outcome monitors for performance, patient satisfaction and clinical care. Education: 1. Serves as a resource and provides information and education to staff, residents and students in areas of formulary management and approved clinical guidelines to reduce unnecessary and inappropriate use of drugs. 2. Educates medical staff regarding formulary management, approved clinical guidelines and the cost efficient use of drugs. Education is provided through in-services and one-on-one encounters. Where possible, uses before and after data to assess the impact of these educational interventions. 3. Provides input and assistance to Pharmacy Administration on issues related to areas of practice and expertise.4. Serves as a preceptor for pharmacy students and residents. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Language: Must be proficient in spoken and written English Education Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. GS-13 GRADE EXPERIENCE Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Preferred Experience: In addition to basic Qualification Standard: completion of ASHP-accredited PGY-1 Pharmacy Practice Residency and/or PGY-2 Specialty Pharmacy Residency (in an applicable specialty) or equivalent experience in the major duties for at least 3 years.
