Overview

 

Within an interdisciplinary team approach, provides psychosocial assessment and intervention for patients in the Weight loss and wellness center, including bariatric surgery patients, medical weight loss patients and diabetes program patients. Such care will include initial psychosocial assessment, provision of education to patients and families, interpretation of clinical assessment, group and individual intervention, follow up care including: reassessment or intervention, and recommendations to treating physician and other interdisciplinary team members as to plan of care for patients. Utilizes department protocols. #cb

 


Qualifications

 

Education:
Master's level degree (e.g., M.P.C., M.S.W., M.S., M.A., M.Ed.) in Social Work, Counseling, Psychology or comparable program required.
Licensure/Certification/Registry:
Current professional license (i.e. LSW, LCSW, LPC, LCPC, etc.) required. LSW and LPC must obtain LCSW or LCPC status within two years of employment.

Experience:
One (1) year experience working with medical patients in a medical or addictions setting preferred.
Other Knowledge/Skills/Abilities:
Proven ability to function within an interdisciplinary team format of care.

 


Responsibilities

 

Completes comprehensive psychosocial assessment of patients as part of an interdisciplinary evaluation of patient readiness for bariatric surgery or medical weight management. Other interdisciplinary components include physical therapy, medical/surgical, nutrition and financial components.
Assessment will combine impressions of informed consent (understanding of bariatric surgery and risks and benefits-when applicable), personal coping style, social/environmental support, educational achievement, history or current substance use behavior, history or current mental health or psychiatric diagnosis and treatment, short and long term goals, challenges to reaching goals and for surgical patients financial resources to comply with post-operative care/guidelines as well as potential impacts of any of these variables on patient adherence to bariatric surgery treatment protocols.
Assessment will include screening for whether further, more detailed, psychological evaluation (including formal testing) by licensed clinical psychologist is needed according to written departmental protocols.
Assessment will culminate in a summary report on these impressions with recommendations. Written report will be completed within 5 business days of assessment date.
Participation in biweekly interdisciplinary team meetings. Professional will come to meetings prepared to summarize assessment findings for patients scheduled for discussion. Professional expected to act as educator to interdisciplinary team members on psychosocial issues and their potential impact on medical treatment adherence.
Completes follow-up assessment of patient coping and adjustment. Follow up assessment will include ability to make lifestyle changes, challenges to making changes, accomplishments, goal evaluation and development. Follows ups will occur as outlined in department protocols and provides intervention (when applicable).
Completes remote patient registration at site of care through Cerner scheduling system.
Completes billing process according to department policy and procedures.
Works closely and collaboratively with the Center's physicians (bariatricians and surgeons) as well as the multidisciplinary team on patient care planning.
Assists patients in identifying community resources to meet mental health treatment needs (when applicable).
