Position SummaryTelework position, can be located anywhere in the US. This position is responsible for member-facing clinical outreach and market engagement to support Medicare Part D Stars initiatives. This position will coordinate, plan, and implement quality initiatives for the clinical pharmacy team supporting Medicare Stars.Responsibilities of this position include:-Oversee and execute clinical pharmacy programs-Coordinate strategies for increasing adherence-Utilize motivational interviewing and health coaching to impact member behavior-Ensure effective collaboration with internal and external matrix partners to support clinical programs-Work collaboratively with leadership and management to strengthen business and operational processes-Provide clinical pharmacy support to programs targeted toward the Medicare membership-Other duties as requiredThis position does involve the potential for travel up to 10%

Required Qualifications-Current unrestricted state pharmacist license in state of residence-Live in and be licensed in a state that allows clinical services to be performed in a work from home position-Doctor of Pharmacy degree from an accredited school of pharmacy-Role includes member outreach all across the country, so work hours may vary based on time zone coverage needed

-Experience with Medicare STARS or managed care

-Significant experience working at a health plan managing Medicare Stars to improve quality outcomes for members. Established experience in functions beyond prior authorization approval or drug utilization review required.

3+ years in a Clinical Pharmacist Role* which must include at least one of the following:-One year of a Pharmacy Practice Residency-Rounding and making therapy recommendations as an active part of a multi-disciplinary care team-Independently managing patient therapy under a collaborative practice agreement

*Clinical Pharmacist Role:-Established experience in functions beyond fundamental dispensing and order-processing activities such as optimization of medication regimen and monitoring or pharmacist-led care management-Experience with population health and/or disease state management-Proven ability to research and analyze medication literature including drug information, disease states, and clinical practice guidelines to identify gaps in care and create/communicate a recommendation plan to providers and patientsPreferred Qualifications

-Bilingual in English and Spanish-Board Certified Ambulatory Care Pharmacist, or Board-Certified Geriatric Pharmacist, or Board-Certified Pharmacotherapy Specialist-Pharmacy residency-Certified in MTM-Knowledge of clinical program components, formulary designs, prescription benefit trends-Previous leadership experienceEducationDoctor of Pharmacy degree from an accredited school of pharmacy

Pay Range

The typical pay range for this role is:

$76,050.00 – $191,100.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
