Job Description

Reporting to the Director of assigned area with accountability to the Pharmacy Manager, the Clinical Pharmacist is responsible for participating in and executing all functions related to clinical pharmacy programs. Responsible to provide pharmaceutical care which provides safe and effective drug therapy to improve patient outcomes.

II. REPRESENTATIVE FUNCTIONS:

Identifies actual and potential medication related problems through review of medication history, patient profile, medical records and interviews with patients and caregivers. Develops and implements monitoring of treatment plans in consultation with other caregivers to correct problems identified.

Provides detailed therapeutic consultation, drug information, education, and literature reviews to medical, nursing, pharmacy and other allied health care professionals. Initiates patient medication education activities when needed

Provides education to physicians, nurses, and other health care professionals, including inservices and lectures. Answers questions and provides guidance for clinical activities of staff pharmacists. Contributes articles for the monthly Pharmacy Newsletter.

Participates in the activities of various Committees, including, but not limited to: preparation of formulary reviews, drug monographs, policies and procedures, and reports as directed by the Director or Manager.

Performs duties as a staff pharmacist in assigned area as necessary; has completed all requirements and training for staff pharmacist position. Demonstrates a working knowledge of current pharmaceutical literature and practice for all age groups of patients served by the assigned area, and makes sound clinical decision based on this knowledge.

III. SPECIAL SKILLS AND ABILITIES REQUIRED:

The communication skills necessary to positively interface with physicians, nurses and other customers.

The interpersonal skills necessary to act as a liaison between the department Director and other departments and areas.

The organizational skills necessary for day-to-day workload management.

IV. KNOWLEDGE, PRACTICAL EXPERIENCE AND LICENSURE/REGISTRATION REQUIRED:

BS Pharm or Pharm D degree.

Current license to practice pharmacy in the State of Illinois.

One or more years of experience in a pharmacy practice site.

BLS and ACLS certification within 3 months of hire required.

V. PHYSICAL REQUIREMENTS:

Light physical effort. May require frequent periods of standing or walking. May require frequent lifting or moving of lightweight materials, occasional moving or lifting of average weight material, or rarely lifting or moving heavy weight material.
