Summary The Patient Safety Clinical Pharmacy Specialist is a licensed pharmacist who plans, coordinates, and manages all pharmacy aspects of medication safety and regulatory compliance at the Jesse Brown VA Medical Center. This individual serves as an active member of the Pharmacy, and Therapeutics Council and related subcommittees. The position reports to the Chief of Quality, Safety, Value and is located at Jesse Brown VA Medical Center, Chicago, Illinois. Responsibilities VA Careers – Pharmacy: https://youtube.com/embed/Fn\_ickNBEws Major Duties: Implementation of patient safety policies and activities developed by the VA National Center for Patient Safety (NCPS). Builds and implements patient safety systems and promotes the culture of safety within VA facilities and for VA patients, families, employees and other stakeholders in all healthcare settings. Works with, trains, and guides a wide variety of collaborators. Responsible for translating the policies, theories, and strategies into action within the complex interrelationships that exist related to patient safety. Training and implementation of the patient safety program, Healthcare Failure Mode and Effects Analysis, (HFMEA) / other proactive risk analyses, and Root Cause Analysis processes to clinicians, management, and all relevant support personnel within their facilities and associated facilities and operations (e.g., home care). Maintaining clear communication between the facility, VISN leadership and NCPS, both to communicate relevant patient safety information and lessons learned from facility which have potential impact beyond the local level and to understand changes at the national level in the Patient Safety Program. Qualifications Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: You must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Today the sole degree is Doctor of Pharmacy.) OR Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination(FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the Basic Requirements above, applicants must meet the following grade requirements. SPECIALIZED EXPERIENCE GS-13 LEVEL: You must have at least one (1) year of experience equivalent to the next lower grade level (GS-12) or equivalent, which is directly related to the Facility Program Manager position which manages all aspects of a distinct program to include policy development, quality resources and specialty functions unique to the program to optimize cost effectiveness (budget) and patient outcomes. Coordinates a single program area of a complex pharmacy operation, or multiple program areas within a facility to develop, organize, manage, and control complex pharmacy programs. Develops, organizes, coordinates and manages single or multiple program areas within the medical facility; this may include a single program area at multiple facilities. In addition, you must fully demonstrate the following KSAs: 1. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. 2. Expert understanding of regulatory and quality standards for their program area. 3. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. 4. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. 5. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
