Sinai Urban Health Institute is hiring a full-time, culturally competent Community Health Worker (CHW) Supervisor. This role is part of the South Side Healthy Community Organization (SSHCO), which is comprised of 13 healthcare organizations including safety net hospitals, health systems and Federally Qualified Health Centers. The model will better connect existing health organizations, increase access to care, address some of the most challenging health issues, and ultimately, build health equity and ensure stronger, healthier communities across the South Side of Chicago.

WHAT YOU WILL DO: The CHW Supervisor provides direction, supervision and mentorship to the program’s CHWs, and continually assesses CHW training needs. They work collaboratively to guide implementation and quality assurance of all SSHCO affiliated CHW interventions. The CHW Supervisor maintains a reduced caseload of patients and provides health education, health system navigation support, and case management services. The position oversees CHWs at locations within the SSHCO and requires travel to sites on the South Side of Chicago. The position is grant funded. SUHI is seeking an enthusiastic, passionate CHW Supervisor candidate who meets job requirements outlined below. WHAT YOU WILL NEED - College degree preferred but not required. Must have either: – Three years of professional experience in a public health, social service or healthcare industry complemented with a BS degree OR – Seven years of professional experience in a public health, social service or healthcare industry without a BS degree. - Works within the multi-disciplinary team onsite at the various locations, develops and maintains relationships with the clinical teams - Assists CHWs in achieving project goals, performs annual CHW evaluations, and program administration - Performs data entry and data management functions for projects as needed - Strong leadership skills in team building and program administration - Strong verbal and written communication skills - Demonstrated commitment to racial and health equity - Proficiency in Microsoft Word, Excel, PowerPoint and Access; working knowledge of medical terminology - Candidates residing in a South Side community are preferred Sinai Urban Health Institute (SUHI) was founded in March 2000 to generate creative approaches to improve the health of Chicago’s urban communities.
