Job Description

Assessment and Evaluation

+ Performs and documents comprehensive assessment; identifies age specific needs and conditions of the patient.

+ Evaluates patient to obtain and interpret data for treatment planning; informs those involved of patient needs and precautions.

+ Initiates referrals for services as dictated by patient’s needs.

+ Identifies discharge planning needs and incorporates these into the interdisciplinary treatment plan.

+ Participates in outcomes measurement data collection and program evaluation.

Planning

+ Plans and documents treatment plan for patient based on evaluation and patient’s age specific needs and conditions.

+ Predicts outcomes and establishes discharge planning needs.

+ Communicates with other team members and adapts treatment according to patient condition.

+ Actively participates in communication with other team members and physicians as needed to develop plan of care.

+ Develops treatment plan in accordance with patient’s individualized and age specific needs, including measurable and functional goals to achieve desired outcomes.

+ Informs team members of patient needs, goals, preferences, and expected outcomes.

+ Collaborates with patient and primary caregiver in working toward treatment goals.

+ Involves patients and family in setting personal goals and encourages patients to make choices about their rehabilitation treatment plan and participation.

Implementation, Evaluation and Education

+ Educates patients and their families on needs related to their condition; reinforces safety precautions at all times.

+ Implements treatment plans and monitors patient response to treatment; reassesses and adapts treatments according to patients’ needs and physician orders.

+ Consults with physicians regarding treatment plans and coordinates treatment plan with all disciplines involved.

+ Coordinates education and uses the appropriate resources to develop and implement an individualized teaching and discharge plan with clients and their families.

+ Discusses with patient and their family evaluation results, progress toward goals, recommendations and equipment; continuously updates patient and family as changes occur.

+ Chooses appropriate intervention based on age and physical needs of the patient.

+ Adheres to hospital infection control standards and techniques.

+ Demonstrates competence in established areas which include but are not limited to; age specific needs of patient, choice and use of modalities, and dealing with abusive/combative patient.

+ Authorized to receive, handle and dispense medications for patient use in the therapy services within the scope of licensure and practice based on a treatment plan developed by a licensed physical therapist.

Position Requirements :

Education: Graduate of approved School of Physical Therapy.

Licensure: Licensed as a Physical Therapist in the State of Illinois.

Certifications: Certified in CPR through the American Heart Association.

Bilingual a plus.
