Description

PURPOSE: The CNA 1:1 Aide is responsible is responsible to ensure the right to courtesy, dignity, privacy, confidentiality and respect of all individuals served and promote independence, personal growth, community participation and integration, economic growth potential and education experiences through facilitation of a variety of meaningful experiences, consistent with choices reflected in the Individual Service/Personal Plans. Specifically, the CNA 1:1 Aide is to provide individualized supports and services focused on a program participant who is funded for 1:1 supports and who requires enhanced supervision, care and engagement to achieve defined outcomes.

ESSENTIAL DUTIES AND RESPONSIBILITIES:The list of essential duties and responsibilities, as outlined herein, is intended to be representative of the task to be performed. The omission of an essential function does not preclude management from assigning duties not listed herein is such duties are a logical assignment to the position.

1. Provide quality care to 1:1 funded program participant and promote overall rights, dignity, and well-being. This can include ensuring specific medical, physical or emotional needs are met.2. Support individuals served to abide by public health guidance related to physical distancing, face covering and personal hygiene3. Support choice making and independence of individuals served through frequent and appropriately styled communication4. Assist in arrival and departure transitions, ensuring safe and efficient movement from one setting or activity to the next.5. Facilitate the engagement of individuals served in a variety of activities by following the area or activity specific schedule. May include a combination of community based and facility based instruction and engagement.6. Implement all objectives of the 1:1 funded program participants care plan and attend to the individual needs of the 1:1 funded program participant which may include daily assistance with personal needs including but not limited to:a. monitoring dietary needsb. following seizure protocolc. incontinence cared. G-Tube caree. tracheotomy caref. ventilator / external oxygen supply careg. hand washingh. transferringi. ambulationj. range of motion7. Provide assistance (defined in the Individual Service Plan, Personal Plan, Implementation Strategy, Positive Behavior Support Plan or Medical Plan and in accordance with cultural and individual preferences) with completion of personal care activities including but not limited to:a. Bathing / Showering (as necessary and possible given the location)b. Hair and skin care (as necessary and possible given the location)c. Use of restroom / toileting, including assistance with catheterd. Oral hygiene (as necessary and possible given the location)e. Perineal caref. Dressing / Undressingg. Mealtime (eating, drinking)h. Body Positioningi. Liftingj. May also assist with the following, using agency approved procedures and equipment:i. Checking temperatureii. Monitoring heart rateiii. Monitoring respiration rateiv. Monitoring blood pressure using agency approved procedures and equipmentk. Supervising / monitoring and assistance with lunch meal servicel. Assisting individuals served in initiation and completion of daily routines/ responsibilities8. Engage in safe and practical use of all adaptive equipment.9. Adhere to responsibilities during emergency drills or actual emergency situations.10. Provide support for unscheduled but urgent or emergent medical care including transportation of the individual/s to & from the care facility, monitoring, oversight and support during provision of medical services, documentation of circumstances leading to visit and outcome and collection & distribution, to appropriate LCF personnel, of relevant guidance provided by care provider.11. Maintain the comfort, privacy, dignity of individuals served in delivery of services.12. Report any changes to the 1:1 funded program participants physical, mental or emotional status.13. Implement behavior management techniques as required when applicable.14. Document, in accordance with program and licensure / certification entities and within the proper system/s, the outcomes of services rendered including but not limited to:a. daily individual or group progress notesb. achievement of defined goals / objectivesc. individual and group attendanced. critical, unusual or medical incidentse. transportation recordsf. other related record keeping15. Provide line of sight supervision for the 1:1 funded program participant and account for the 1:1 funded program participant before, during and upon return from all activities & locations.16. Serve as an appropriate role model for persons served maintaining positive work practices regarding attendance, dress for job performed, punctuality, cleanliness of work area, use of equipment, etc.17. Maintain a positive working relationship with staff, persons served, families/personal representatives, sponsoring agents, etc.18. Assist with transporting individual(s) as a rider as needed to maintain the safety and security of individual(s) on agency or other designated vehicles.19. Advocate for the individual(s) by assisting with communication to address concerns and issues which may be a barrier to independence.20. Communicate need for additional / replacement program supplies necessary to execute activities and personal care.21. When providing services which require transportation, follow all safety procedures (proper use of safety/seat belts or tie-downs, driving practices, vehicle checks)22. Prepare individual served for transitions between scheduled classes, programs or activities.23. Provide direct (in person and virtual), appropriate and approved engagement opportunities following a prescribed schedule created in accordance with the Individual Service/Personal Plans of individuals served.24. Implement associated / assigned LCF CDS curriculum & adhere to schedules.25. Facilitate accomplishment of personal goals and objectives of individuals served through adherence to Individual Service Plans, Personal Plans, Implementation Strategies / Positive Behavior Support Plans or Medical Plans.26. Communicate feedback and changes, in a manner consistent with the support needs of all persons served, frequently – ensuring consistency and allowing for adequate processing of instruction.27. Provide feedback to agency representatives which furthers development of functional outcomes and program enhancements related to individuals served.28. Implement Positive Behavior Support strategies, and document as required.29. Abide by all agency policy and procedure related to maintaining the health, safety and welfare of individuals served.30. Successfully completes the Medication Administration Training and maintains the ability to pass medications.31. Administer emergency or immediate first aid care and techniques during an emergency and/or quickly respond to accidents to ensure that injuries or illnesses are efficiently and promptly dealt with before a trained medical professional arrives or until appropriate personal arrive.32. Support choice making and independence of individuals served through frequent and appropriately styled communication.33. Engage in safe and practical use of all adaptive equipment34. Adhere to responsibilities during emergency drills or actual emergency situations35. Ensure organization and cleanliness of the assigned area / location / site and report any safety hazards, accidents or other barriers to promote a positive environment and a safe workplace.a.
