Job Description
Looking for a position where you can make a difference? CTF Illinois is an essential employer seeking a Case Manager (QIDP) to support individuals with developmental disabilities. Overview:
- Work schedule: Monday through Friday generally
- Submitting DHS required documentation in a timely manner
- Conduct home visits quarterly
- Coordinate and Implement an Individual Service Plan (ISP) for individuals receiving services by following the recommendations made by medical and professional consultants
- Assist individuals with completing budgeting, shopping and other money management issues
- Coordinate with other departments including, but not limited to: Nursing, CILA, and Day Program
- Monitor each individual’s progress by completely monthly reviews and maintaining each individual’s personal history book
- As required by DHS, complete an additional 12 hours of continuing education units per year, at least 6 of which will be provided from outside the agency
Requirements:
- Bachelor Degree from a four-year college or university in a human service related field
- Meet requirements for certification and training as a QIDP
- Minimum 1-year experience working with individuals with developmental disabilities
- Valid driver’s license with acceptable driving record per company requirements
- Ability to work independently and prioritize work in timely manner
- Certification and training as QMRP/QIDP preferred
- Previous DSP training preferred
