This will be a working leadership position that work with our Assertive Community Treatment Team, a multidisciplinary team working with individuals with severe and persistent mental illness and dual disorders living in the community. This position will provide services in an array of settings including but not limited to, an individual's home, hospital, emergency room, medical providers or other social service agencies. This position will provide nursing oversight for the LPNs working on the team.
Responsibilities Include:
Care Coordination
Coordinate and attend healthcare appointments
Consult with medical providers
Administer medications (oral and injections)
Provide education to individuals with severe and persistent mental illness
Provide education and guidance to co-workers related to person's served healthcare needs
Medication Management
Advocacy and Oversight of healthcare needs
Medication reconciliation for all individuals served
Primary point of contact for pharmacy and providers working with the team
Provide supervision and support to ACT and Williams nursing staff
Facilitate medication transfer/reconciliation for class members/individuals served transitioning to ACT services
Other duties as assigned.
Education and Experience:
Ideal candidate will have the ability to work independently yet within a collaborative and team based approach. Successfully applicants will possess a current and undisciplined State of Illinois Registered Nurse licensure. Previous supervisory experience in a healthcare setting is highly recommended.
Valid driver's license, reliable means of transportation and proof of current automobile insurance are required.
