Job Summary
In collaboration with an Endocrinologist and under the administrative supervision of Chief of Endocrinology, the Endocrinology APN provides exceptional care to a diverse patient population in areas of endocrinology and diabetes. Promotes wellness and health maintenance by performing comprehensive diagnostic and therapeutic medical services. Does triage activities in Diabetes Center and Diabetes Quality Assurance supervision for in-patients with diabetes. Develops and implements patient management plans in providing continuity of care.Manage endocrine diseases in an outpatient setting. Provides these activities throughout the Cook County Health (CCH) clinical sites. Maintains professional and technical knowledge through continued educational programs and workshops supporting emerging healthcare trends.Serves as a nursing resource, clinician, educates students, and participates in research and development. Coordinates procedures that require a multi-disciplinary approach.Performs duties within the legal guidelines of CCH policies.
Typical Duties
*Provides and manages direct patient care in areas of endocrinology and diabetes.
*Conducts patient's health needs assessments
*Obtains comprehensive history and performs detailed physical examinations to determine accurate diagnosis
*Orders appropriate diagnostic tests or lab work necessary, i.e. x-ray, magnetic resonance imaging (MRI), endocrine tests, thyroid biopsies and dynamic endocrine tests, etc.
*Informs and effectively communicates with patient test results and/or diagnosis
*Identifies and communicates with supervisor any patient care issues; offers research guided recommendations for improvement initiatives if applicable
*Develops and implements patient management plans in providing continuity of care
*Performs procedures, treatments, quality improvement and administrative activities and other subspecialty dependent modalities within the scope of the supervising physician to improve patient outcomes
*Adheres to quality and patient safety standards set forth by CCH
*Adheres to infection control policies and protocols, i.e., hand hygiene
*Prescribes and dispenses medications utilizing best practices
*Complies with federal, state and local legal and professional requirements.
*Maintains professional licensing requirements and technical knowledge through continued educational programs and workshops supporting emerging healthcare trends

 

Minimum Qualifications
*Licensed as a Registered Professional Nurse in the State of Illinois
*Licensed as an Advanced Practice Nurse-Nurse Practitioner in the State of Illinois or ability to obtain license prior to starting employment
*Master's degree or higher in Nursing from an accredited college or university
*Certification by a nationally recognized body as designated by the rules and regulations of the Nurse Practice Act
*Current DEA license or the ability to obtain license within six (6) months of hire
*Current IL controlled Substance License or the ability to obtain within six (6) months of hire
*Minimum of (1) one year progressive experience and advanced knowledge in management of complex diabetes patients as an APN
*Basic Life Support (BLS) Certification through the American Heart Association
*Must be able to travel work sites to provide outpatient care
Preferred Qualifications
*Doctoral degree from an accredited college or university
*Prior experience and knowledge in use of complex diabetes medical regimens including various types of insulin and all available oral diabetes and hypertension agents
*Prior experience and knowledge of use and management of computerized database
*Certified Diabetes Educator (CDE)
*Completion of a one (1) year Cook County Health APRN fellowship
Knowledge, Skills, Abilities and Other Characteristics
*Knowledge of proper hand hygiene and hand washing techniques
*Knowledge of infectious control policies and procedures
