JOB SUMMARY: NNOC MEMBERS ONLY LOCATION: John H. Stroger, Jr. Hospital DEPARTMENT: Critical Care - Four Flex SHIFT: 7:00 P.M - 7:30A.M. Starting salary is commensurate with years of RN experience The Clinical Nurse I is a registered professional nurse provider who is responsible and acceptable for providing safe patient-centered nursing care for the Intermediate Care patients. The Clinical Nurse I applies knowledge of the nursing process to implement nursing practice and perform technical skills for patient care delivery. Competent nursing practice is accomplished through the application of scientific principles of nursing, problem solving skills and demonstrated teaching/learning experiences. Typical Duties Performs admission ad on-going patient assessment Performs admission assessment on assigned patients upon arrival to the Intermediate Care Unit. Obtains assessment information from patients/significant others as appropriate. Collects assessment data as a basis for formulating nursing diagnosis and provide appropriate age specific care relative to the following factors: biophysical, psychosocial, and environmental, self-care capabilities, safety, infection control, learning needs and ability to manage continuing care needs after discharge. Performs ongoing and scheduled reassessments according to divisional / unit standards. Performs transfer and discharge assessments on assigned patients when transferred/ discharged from the department Demonstrates knowledge of decontamination process. Creates a plan of care based on assessed needs Establishes nursing diagnosis, desired outcomes, and interventions from collected assessment data. Collaborates with patient, significant other, and other health care team members in planning care during hospitalization. Collaborates with patient, significant other, and other health care team members for anticipated discharge needs. Develops nursing plan of care that is consistent with the therapies of other disciplines, is based on patient age relative to biophysical, psychosocial, environmental, self-care capabilities, safety, Infection control, learning needs and * discharge planning needs. Communicates with patient and significant others the nursing plan of care. Implements the nursing plan of care. Performs interventions in a competent, skillful and safe manner. Prioritizes interventions according to patient needs. Applies principles of growth and development across the life span in providing care appropriate to the age-specific needs of the neonate / infant, child, adolescent, adult and geriatric client. Provides education to patient and significant other based on assessed learning needs. Performs the technical aspects of care according to established practice standards. Demonstrates competence in the safe and effective use of clinical equipment. Performs or directs cardiopulmonary resuscitation and other lifesaving interventions. Demonstrates competence in applying the principles of BSIS, isolation and asepsis. Evaluates effectiveness of care and revises interventions as appropriate. Evaluates patient's responses to interventions performed and document appropriately. Modifies and implements interventions as appropriate. Re-evaluates patient's response. Documents appropriately. Records accurately, concisely, and legibly in black ink or in the electronic medical record the following information: assessment, reassessment, plan of care, interventions and patient's responses. Signs documentation with legible signature and classification. Demonstrates leadership ability. Performs charge nurse/ team leader responsibilities competently when assigned. Demonstrates effective communications. Demonstrates ability to communicate accurately, dearly and concisely, orally, written and electronically. Demonstrates caring and respectful attitude in verbal and nonverbal communications. Communicates pertinent information to patient, physician, nurses, and other healthcare team members as appropriate.
