Gather a comprehensive database and perform initial and ongoing assessment of assigned patients’ medical, physical, and psychosocial status: a. Conduct and document histories and physicals b. Order and evaluate diagnostic tests, i.e., lab works, x-rays, EKGs, consults, and similar studies c. Develop treatment plans in collaboration with other attending physicians and health care team. d. Record progress notes, write discharge notes and summaries e. Generate appropriate referrals to other health care professionals and community agencies according to established procedures 2. Discriminate between normal/abnormal findings and formulating a treatment plan accordingly. Communicate with attending physicians regarding abnormal findings. 3. Initiate and adjust selected medications with formulary approval for inpatients. 4. Document in the medical record all procedures performed for which the Staff Urologist is responsible. Institute and adjust plans of care, utilizing Staff Urologist skills. 5. Provide appropriate response to life-threatening emergency situation when no physicians are immediately available. 6. Document clinical findings and progress notes in the medical record. 7. Develop and provide patient treatment and education plan. 8. Initiate and monitor health maintenance in conjunction with the patient, family/significant other and members of the multi-disciplinary team. 9. Provide and promote teamwork support and collaboration with multi-disciplinary team members. 10. Participate in interdisciplinary team conferences to assure the quality patient care management. 11. Determine, in collaboration with Attending physician, appropriate admissions, discharges referrals and transfers to different levels of care and coordinates management of patients. 12. The incumbent will be responsible for ensuring that the care provided meets or exceeds VA, Department of Defense, and external standards. 13. The incumbent provides care to outpatients and inpatients within the VHA system. The incumbent uses the Computerized Patient Record System (CPRS) for physician order entry (prescriptions, laboratory and radiology orders, consult requests) and notes, reviews of laboratory, radiology, procedure and consultation reports and communicates abnormal results to patients. 14. The incumbent performs admission history and physical examinations, discharge notes and summaries, advance directive, code status documentation, progress notes, Abnormal Involuntary Movement Scale (AIMS) evaluations, rounds, case reviews with residents and mid-level providers, and attends discharge planning and treatment team meetings. All encounters are expected to be accurately coded. 15. The candidate with work as equally clinical partner with the existing Urology Surgery team, responsible to create, publish and share equally in the on-call schedule to ensure the facility has 24/7 coverage for urologic emergencies 16. Will participate in all aspects of the urology surgery program, to include: providing comprehensive surgical care services within the scope of practice of a urologic surgeon, to provide appropriate follow-up care of existing conditions, to manage emergency conditions, to provide pre-operative and post-operative patient care, including daily hospital rounds as indicated, and to provide inpatient and consult service during off tours (nights, weekends, and holidays) as required by the Directorate, the VHA, and as assigned, and aligned with Section resources. Work Schedule: Monday-Friday, 7:30am-4:00pm Requirements Conditions of Employment U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Qualifications To qualify for this position, you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement. Applicants pending the completion of training or license requirements may be referred and tentatively selected but may not be hired until all requirements are met. Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or Osteopathic Medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR [(2) Those approved by the American Osteopathic Association (AOA),OR (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences.
