Job Summary: Assures the appropriate selection, dosing, administering and dispensing of medication orders. Provides supervision for actions of pharmacy technicians, residents, graduates and interns. Interacts with patients, physicians, and nurses concerning various clinical activities in order to provide proper drug therapy. Functions as Pharmacist in Charge (PIC) in the absence of the Pharmacy Supervisor/Manager. Job Responsibilities and Requirements: PRIMARY RESPONSIBILITIES Verifies prescription orders and dispenses medications as ordered by ensuring the accuracy of entered prescription orders, by performing DUR/DUE, verifying drug/drug interactions and completing visual verification of orders. Transfers prescriptions to and from other pharmacies following state and federal law requirements. Compounds prescription medications when necessary and assures appropriate documentation is completed. Ensures Medication Guides, CMS notices and other documents required by law are distributed with prescriptions. Monitors medication therapy and makes recommendations to therapy when appropriate. Counsels as required by all state and federal laws and provides information on drugs, their pharmacological action, dosage, contraindications, reactions, etc. to physicians, nurses, patients, and other interested parties. Identifies and completes medication therapy management opportunities and recommends, administers, and educates on immunizations as appropriate. Stays current with professional literature and maintains competence required for position. Oversee technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies. Able to perform all duties of a pharmacy technician. Assist with academic functions such as serving as a preceptor for pharmacy students. Maintain appropriate inventory of medications and supplies. Ensure inventory is rotated, expired stock is appropriately disposed of and orders are placed in a timely manner. Provide support and serve as a resource for nurses, physicians and other health care providers medication information and education. Collaborate with leadership to ensure policies and procedures are reviewed and makes recommendations for improvement. Actively assesses the security of medication areas and the compliance of personnel with established policies, procedures, state and federal laws including record keeping. Maintains established procedures for quality control and disposal of unusable medications and hazardous waste. Issue controlled substances and maintains records as required by law, pharmacy policies and procedures. Reviews controlled substances records for discrepancies/errors and reports discrepancies to appropriate authority(s). Operate cash register, including end of day check out, with minimal shortage/overages. Ensures all insurance claims are processed according to policy and procedures. Confirms accuracy of prescription billing, days’ supply, quantity, MME, source and DAW to avoid audit charge backs and ensure compliance with Fraud Waste and Abuse (FWA) Laws. Contributes to zero medication errors with injury by completion of Near Miss and Occurrence reports. Assists pharmacy supervisor in reviewing medication event reports and identifying and implementing process improvement opportunities. Provides excellent customer service, counsels patients on drug information with a high standard of confidentiality and in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
