Clinical Nurse Liaison

SUMMARY: The primary purpose of this position to is to coordinate smooth transition of residents into long term care facilities. The individual will meet with families and caregivers to explain care delivery, and will collaborate with discharge planners and physicians for the entry of patients into a facility. ESSENTIAL DUTIES AND RESPONSIBILITIES:

+ Provide educational in-services and other educational opportunities to the community and providers in assigned territory annually at minimum.

+ Responsible for census development and growth of long-term care inpatient care.

+ Expedite transition from hospital to home by obtaining signed consents at the hospital.

+ Manages a group of assigned hospitals, LTC facilities and ALF facilities.

+ Responsible for managing the outreach activities for an assigned coverage area on a daily basis.

+ Meets with appropriate physicians in the service area who influence or make referral decisions.

+ Meet with appropriate physician office staff nurses to familiarize them with the patient characteristics.

+ Meet with assigned hospital discharge planners and floor nurses to establish a positive referral relationship.

+ Develop opportunities to conduct in-service presentations for physician offices, hospitals, nursing homes, assisted living and other health care providers.

+ Coordinates utilization review with assigned providers of care.

+ Review patient’s hospital medical chart and complete a bedside evaluation for assigned patients.

+ Review and evaluate skilled level of care for each assigned patient including review of medications and equipment required.

+ Complete clinical admission of when admitted to facility.

+ Works closely with the facility leadership, Clinical Managers, and all Team Directors to insure that promised services are provided.

+ Performs other duties as assigned.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required. · Registered Nurse or Licensed Practical Nurse with current unencumbered state licensure. · M ust be able to read, write, speak, and understand the English language. · Must be able to make independent decisions when circumstances warrant such action. · Must be knowledgeable of nursing & medical practices & procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities. · Experience in Long Term Care is preferred. PHYSICAL DEMANDS: The physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this job.
