Carmel office, covering Wayne and Edwards Counties with the option for remote work in Wayne and Edwards Counties. This position reports directly to the Care Coordinator Supervisor. Responsibilities
- Coordinates services for older adults to remain independent.
- Determine eligibility and create care plans for Illinois Department on Aging
- Educate on resources, options and provide case management to older adults and their families
- Requires a BA/BS in Social Work or Human Service related field, or RN
- Equivalent years of related experience with older adults may substitute for college degree requirements.
- Must have valid driver’s license and transportation
- 12 vacation days earned in the first year
- 11 paid personal days/holidays per calendar year
- 6 sick days earned per year
- Retirement plan with 3% company contribution after becoming eligible in the plan (a matching contribution by the employee is not required to receive this benefit). Additional profit-sharing contributions to the retirement plan are possible. Employee deferrals to the 401(k) and Roth plan available
