Job Description
- Performs admission ad on-going patient assessment
- Performs admission assessment on assigned patients upon arrival to the Intermediate Care Unit. Obtains assessment information from patients/significant others as appropriate.
- Collects assessment data as a basis for formulating nursing diagnosis and provide appropriate age specific care relative to the following factors: biophysical, psychosocial, and environmental, self-care capabilities, safety, infection control, learning needs and ability to manage continuing care needs after discharge.
- Performs ongoing and scheduled reassessments according to divisional / unit standards.
- Performs transfer and discharge assessments on assigned patients when transferred/ discharged from the department
- Demonstrates knowledge of decontamination process.
- Creates a plan of care based on assessed needs
- Establishes nursing diagnosis, desired outcomes, and interventions from collected assessment data.
- Collaborates with patient, significant other, and other health care team members in planning care during hospitalization.
- Collaborates with patient, significant other, and other health care team members for anticipated discharge needs.
- Develops nursing plan of care that is consistent with the therapies of other disciplines, is based on patient age relative to biophysical, psychosocial, environmental, self-care capabilities, safety, Infection control, learning needs and · discharge planning needs.
- Communicates with patient and significant others the nursing plan of care.
Implements the nursing plan of care.
- Performs interventions in a competent, skillful and safe manner.
- Prioritizes interventions according to patient needs.
- Applies principles of growth and development across the life span in providing care appropriate to the age-specific needs of the neonate/infant, child, adolescent, adult and geriatric client.
- Demonstrates competence in the safe and effective use of clinical equipment.
- Performs or directs cardiopulmonary resuscitation and other lifesaving interventions.
- Demonstrates competence in applying the principles of BSIS, isolation and asepsis.
Minimum Qualifications
- Licensed as a Registered Professional Nurse in the State of Illinois or ability to obtain license two (2) weeks prior to starting employment (must provide proof at the time of interview if already obtained)
- Basic Life Support (BLS) certification (must provide proof at the time of interview if already obtained)
- Current Advanced Cardiovascular Life Support (ACLS) certification or ability to obtain prior to conclusion of orientation (must provide proof at the time of interview if already obtained)
Preferred Qualifications
- One (1) to five (5) years of acute care experience
- CCRN, PCCN, NIH Stroke, or other specialty certification
