Number of Positions: 3
Starting Salary: $82,488.00
This position is open to the public and all current City employees covered under the City’s collective bargaining agreement with ILLINOIS NURSES ASSOCIATION – BARGAINING UNIT 37. Only employees in City job titles in this bargaining unit are eligible to bid.

Mission: Chicago Department of Public Health (CDPH) works with communities and partners to create an equitable, safe, resilient, and healthy Chicago.
Under supervision, the class works primarily in the field, providing professional nursing care to individuals, groups, and families to improve the overall health of communities, or functions as a nursing team leader in a health clinic, and performs related duties as required.
ESSENTIAL DUTIES
· Educates clients and communities about public health concerns
· Works with a caseload of clients in community settings
· Assesses client and community needs
· Works with community health providers to address community health problems
· Coordinates the delivery of appropriate client services
· Participates in case conference/consultations to share challenges/successes
· Documents information in relevant data collection systems
· Works with a multidisciplinary team, including Infection Preventionists, Medical Directors, and Epidemiologists to assist in outbreak response and prevent healthcare-associated infections
· Participates in field visits to healthcare settings to:
o Review practices and assess risk to patient safety
o Collect clinical case information and review medical records
o Assists with specimen collection in the field (e.g., nares, skin, rectal swabs)
o Develop training materials and conduct training for healthcare staff (Healthcare Settings Program PHN II)
· Contacts families to set up home visits, provide education to families on lead poisoning and prevention, conduct developmental and social emotional screenings, visual assessment of home, discuss follow up care with families and medical professionals, collaborate with Building Construction Inspectors to determine lead hazards (Lead PHN II)
· Contacts families to set up home visits, conduct home visits or telehealth visits to collect data, complete maternal, infant, and family well-being assessments, and ensure clients are connected to referral sources (Family Connects PHN II)
· Provides directly observed therapy and case management to persons diagnosed with tuberculosis (TB PHN II)
Additional duties may be required for this position
Location: Various City-wide locations
Days: Monday – Friday
Hours: 8:00am – 4:00pm, or 8:30am – 4:30pm, or 9:00am – 5:00pm

Qualifications:
MINIMUM QUALIFICATIONS
· Graduation from an accredited college or university with a Bachelor’s degree in Nursing accredited by the National League for Nursing (NLN) or an approved foreign credential evaluation, plus one year of professional nursing experience
Licensure, Certification, or other Qualifications
· Must be licensed to practice as a Registered Nurse
· Applicants must possess a valid State of Illinois Registered Nurse license at the time of employment
SELECTION REQUIREMENTS
This position requires applicants to complete an interview which will include a skills assessment as part of the interview. The interviewed candidate(s) possessing the qualifications best suited to fulfill the responsibilities of the position, based on the oral and written parts of the interview will be selected.
Preference will be given to candidates possessing the following:
· Previous experience working with diverse populations
· Previous experience working in a clinical setting
· Bi-lingual proficiency
· Familiarity with the IDPH lead code and families with children under 6 years of age (Lead PHN II)
· Previous experience in maternal/child health and home visiting (Family Connects PHN II)
· Previous experience in infectious disease (TB PHN II)
· Proficiency working with Microsoft Office Suite
