What you will need:
Requires independent decision-making and clinical judgment skills, with in-depth knowledge of oncology and therapeutics.
Two years experience in hospital pharmacy and successful experience in preparation of chemotherapeutic agents or bio-tech products.
Ability to think, plan, and act strategically
Effective oral and written communication skills
Working knowledge of and familiarity with the hospital and cancer center environment
Computer Skills: proficient in software (MS Outlook, Word, Excel), comfortable with automation system, and embraces technological change
Currently licensed by the Illinois Department of Professional Regulation (of Education and Registration)
Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited Doctor of Pharmacy degree program
PGY1 and PGY2 (in the area of specialization) Residency or the equivalent experience
Board Certification in Oncology preferred

JOB SUMMARY

Interprets physician orders in relation to the patient's oncologic diagnosis, prescribed treatment, age group and makes appropriate interventions as needed to assure safe and effective drug therapy. Enters or verifies medication orders into the computer, admixes or dispenses medication (including chemo) for patients as prescribed by the physician, and practices and promotes the concept of patient-focused care.

Participates in Drug Utilization Evaluations (DUEs), drug review, education, patient monitoring, Quality Assurance (QA) programs and drug information activities of the oncology pharmacy.

RESPONSIBILITIES

Essential Job Functions

Develops and maintains a clinical practice by providing patient care service and cooperating with medical and nursing personnel to optimize the pharmacotherapeutic aspects of patient care. Provides optimal selection of pharmacologic agents, designing a regimen to maximize therapeutics benefit and minimize toxicity. Provides daily follow-up on the clinical effects of the regimen with adjustments as necessary to achieve desired outcomes.

Provides drug and clinical information to other healthcare professionals in response to specific questions on newly evolving issues related to the care of patients with cancer. Provides professional education regarding new drugs, biologics, or technologies for specialized drug delivery. Provides a significant level of education to professionals, trainees, and patients. Responsible for monitoring and assessing performance of the learner and providing feedback using appropriate learning objectives that are aligned with the desired learning outcomes.

Provides visual, spoken, and written clinical education, training, and feedback to pharmacy students, pharmacists, clinical pharmacists, and residents in the Department of Pharmacy.

Provides managerial responsibilities for the operations of the Oncology infusion center, including process improvements, completing performance reviews for pharmacy staff if any, and maintaining the oncology formulary. Orders medications and maintains inventory of the oncology pharmacy.

Designs, supports, and implements policies to provide effective and efficient use of resources ensuring that budget, quality, and value targets are met in collaboration with the Director of Pharmacy.

Develop, review, assist, and maintain, in collaboration with members of the Department of Hematology/Oncology, various protocols and regimens within the oncology electronic system.

Participates in committees including, but not limited to: Pharmacy Steering Committee, Chemotherapy/Biotherapy Sub-Committee, Pharmacy and Therapeutics Committee, and, Hematology/Oncology staff Meetings.

PATIENT CARE/AGE SPECIFIC RESPONSIBILITIES AND QUALIFICATIONS

On a daily basis, profiles medication orders via computer accurately and /or makes appropriate changes or additions on the patient profiles, admixes and dispenses chemotherapy medication, and attends rounds in the clinic areas with direct patient care contact for visual assessment of physical and psychological signs of drug-related effects.

Performs pharmacokinetic dosing and monitoring for all patients on drugs where therapeutic levels are reported.
