Job Summary:
Under the guidance of the Clinical Supervisor and local market leadership, the Registered Nurse – Care Coordinator collaborates with local and market teams to:
+ Assess, develop and revise action plans for Help at Home clients.
+ Respond to client needs as identified by Help at Home caregivers and others and identify community, payer and provider resources to meet those needs.
+ Collaborate with internal and external stakeholders and workgroups to identify areas of need and opportunity across the organization.
This position is 80% remote and 20% field based to support our clients throughout Cook County, Illinois. Candidates must live in or within driving distance of Cook County, Illinois to be considered for this role.
D uties & Responsibilities:
+ Reviews client needs/referrals as identified by caregivers and others, conducts additional assessments, and develops a service plan to address those needs. Ensures appropriate follow up occurs.
+ Performs telephonic and in-person assessments of clients based on nursing standards of practice
+ Communicate findings as needed to the clients’ primary care physician and/or specialists
+ Participates as a key stakeholder in Quality Improvement activities
+ Considers the physical, psychological and socioeconomic needs of the client and family and the ability of client needs to be met in the home
+ Documents assessments and service plans timely and accurately
+ Educates client, caregiver and family as appropriate and provides information on available community resources
+ Performs other related duties as assigned
Skills & Abilities:
+ Ability to work independently and leverage community resources to benefit clients
+ Excellent interpersonal, written, and verbal communication skills
+ Energetic team player with a positive attitude and curious mindset
Education & Experience:
+ Graduate of an accredited nursing school. BSN preferred.
+ Three (3) years clinical nursing experience, home care background is a plus
+ Illinois RN licensure
+ Experience working with medically fragile clients, addressing medical, social and behavioral components of care
+ Case Management or Care Coordination background preferred
