The Social Worker provides guidance, assessments, support, and information to patients and their families to maximize the psychosocial functioning and adjustment of the patient in chronic kidney disease, including but not limited to, information and support related to behavioral health services, chemical dependency, coping skills, adjustment issues, bio-psycho-social support, and community resources

Essential Duties & Responsibilities:

Complex Care Management
Conduct patient assessments, screenings, and follow up for patients and/or caregivers facing high risk and/or complex behavioral health needs. Offer longitudinal care management and behavioral health support. Evaluates and monitors ongoing psychosocial issues and patient goals. Counsels new and current patients and families on treatment options and modalities; provides support, information, and assistance in treatment planning and adjusting to dialysis

Consult
Communicates and consults with interdisciplinary team regarding patient psychosocial issues affecting treatment and well-being. Offer office hours and collaboration with team to offer subject matter expertise. Engages in complex case reviews with the internal team.

Coordinates
Independently analyzes and navigates referrals to community, private, state, and federal agencies based on psychosocial needs (e.g. mental health, financial programs, medication assistance programs, vocational rehabilitation programs, transportation resources, and home health agencies).

Administrative
Other duties as assigned, operational meetings, teammate trainings, and administrative tasks

Qualifications:
Master's degree in Social Work (MSW)
Licensed or Registered in the state of practice as required by state regulations
Licensed masters-level social worker or Licensed clinical-social worker
Demonstrated knowledge of government and private insurance programs
Intermediate proficiency in MS Work, Excel, PowerPoint, and Outlook Experience working with kidney care patients preferred
