Join us!The hospice aide is a member of the interdisciplinary group who works under the supervision of the RN Case Manager and performs various services for a hospice patient as necessary to meet the patient's personal needs and promote comfort. The hospice aide is responsible for observing the patient, reporting these observations, documenting observation and care performed.

Job Description
In this role the Hospice Aide will:
Perform personal care needs including, but not limited to, bathing, back rubs, oral hygiene, hair care, change bed lines, assist with feeding, in accordance with Patient Plan of Care. If unable to perform a task, report to Case Manager immediately and document accordingly.
Meeting safety needs of patient and using equipment safely and properly.
Provide proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence.
Assess and report on patient's condition and significant changes to the Case Manager.
Deliver supplies to patient as needed.
Complete documentation accurately, timely and in accordance with agency policy.
The ideal candidate will have:
High school graduate or equivalent.
Current CNA certification in IL.
Ability to communicate effectively (verbal and written, English) with residents and staff.
Genuine compassion and patience to senior adults.
