Division Overview: Youth & Family Services (YFS) help parents and young people who are striving to become healthy and restore their relationships. YFS works in partnership with human services, child welfare systems, juvenile courts, and community-based service providers so that together we can help individuals and families obtain the services and skills they need to live in positive, healthy ways.

We are currently looking for full-time – Case Manager

Starting at $45,000 – contingent upon experience, education, etc.


Position Summary: TASC Case Manager will provide comprehensive and specialized case management services for adults and youths with identified Behavioral Health needs referred to TASC from multiple systems. TASC Case Manager will provide direct services and referrals based on identified needs of the individuals enrolled into TASC services. TASC provides case management services Statewide and location is determined by work site location.

Essential Duties and Responsibilities:

  • Assist individuals in achieving service plan goals through direct services and coordination of behavior health, public agencies, and community resources.
  • Assess, evaluate, facilitate, and monitor individuals' progress in relationship to established goals and objectives
  • Complete intake packets for program enrollment and participation
  • Provides referrals and crisis intervention for the individuals' identified needs
  • Interview individuals, review records, and confer with other professionals to review individual progress, and to determine their suitability for program participation
  • Knowledge of DSM V and ASAM criteria
  • Collaborate with internal and external partners, participate in staff meetings and monthly supervisions, and participate in integrated staffing with individuals receiving TASC services.
  • Identify and refer individuals with behavioral health needs
  • Complete and submit internal and external program reports
  • Complete and maintain all required individual and program related documentation which includes, documenting the results of screenings, enrollments, service plans, reports, progress notes, discharge summaries and other individual-related activities including accurate reporting of services
  • Monitor all individuals determined by individual progress towards service goals
  • Monitor TASC database system and contractual/external database systems to ensure Individual enrollment
Requirements:

Our Ideal candidate:

  • Bachelor's degree preferred and/or three years' lived or professional experience.
  • Ability to obtain CADC/CARS certif
