Job Description

Position Overview: The Behavioral Health Care Coordinator plays a critical role in ensuring client access to behavioral health services. This individual will be integral to seamless behavioral health program operations, client interactions, and interdisciplinary coordination. The ideal candidate has advanced interpersonal engagement skills and is highly organized, diligent, and committed to improving client outcomes. 

Responsibilities:

  1. Client Engagement and Coordination:
    • Informed Consent: Thoroughly review Behavioral Health Department policies and obtain informed consent for all clients referred for BH screening
    • Intake and Screening: Conduct behavioral health screening assessments in order to determine level of urgency and the nature of clinical services required
    • Referrals: Provide referrals and schedule appropriate services with clinical team providers (i.e. psychiatry, mental health, substance use, primary care) 
    • Assessment Appointment Attendance: Assist in ensuring successful client attendance at clinical assessment appointments (e.g. coordinate with case managers, contact clients directly, provide reminders, etc.)
    • Barriers Clearance: Identify barriers (e.g., transportation, financial, child care) that may hinder clients from attending appointments and develop ideas/processes to assist in circumventing these challenges 
  2. Program Operations: 
    • Interdisciplinary Conferences: Lead meetings with clinicians and other professionals to discuss client progress and treatment plans. 
  3. Client Follow-Up and Retention:
    • Client Tracking: Maintain a system to track referrals, client attendance, follow-up appointments, and treatment adherence
    • Intervention and Outreach: Proactively reach out to clients who miss their initial assessment appointments
  4. Collaboration and Leadership:
    • Team Collaboration: Work closely with clinicians, case managers, and administrative staff to ensure seamless service delivery

 

  1. Other Duties as Assigned
    • Conduct clinical mental health assessments (IM+CANS) and develop client treatment plans as needed according to agency standards
    • Conduct individual and group clinical services as needed

 

Qualifications:

  • Education and Licensure:
    • Minimum of a Master’s degree in Social Work (MSW), Counseling, or related field 
    • Additional certification (e.g., CADC) related to substance use services is a plus
  • Experience:
    • Minimum of 3-5 years of experience delivering behavioral health or substance use services
    • Prior management experience preferred
    • Familiarity with evidence-based practices and quality improvement processes
    • Experience in administering IM+CANS preferred
  • Skills
    • Strong communication and interpersonal abilities 
    • Excellent organizational skills and attention to detail
    • Ability to work independently and take initiative
    • Proficiency in electronic health records (EHR) systems
