Hours: Monday-Friday, 8:00am -4:30pm

What you will need:
Bachelor of Science degree
Ability to communicate effectively with a diverse patient population
Working knowledge of MS Office applications (Word, Excel), intranet/internet, and web-based portal functionality
Ability to chart and explain patient needs through assessment
Ability to interact positively and effectively with multiple departments
Ability to use good judgement and utilize independent decision-making skills to effectively problem solve and resolve issues
Fluency in Spanish, Urdu, Hindi or Arabic a plus.
EPIC experience preferred
Current Illinois state license as a Licensed Dietitian Nutritionist
Current registration as a Registered Dietitian by the Commission on Dietetic Registration
Current certification by the Certification Board for Diabetes Care and Education as a Certified Diabetes Care and Education Specialist (CDCES) preferred

JOB SUMMARYThis is a temporary position for 1 year, grant funded.
The Outpatient Registered Dietitian plans, directs, and provides nutritional care via the Nutrition and Diabetes Center at Swedish Hospital. Provides individual and/or group medical nutrition therapy and/or diabetic education to patients and families or significant others to improve nutrition status and diabetes management. Conducts outpatient Indirect Calorimetry measurements as needed. Works directly and collaboratively with Nutrition Department, including inpatient, outpatient, and community Registered Dietitians. Communicates clearly with interdisciplinary team.

RESPONSIBILITIES
Essential Functions

Demonstrates a service orientation and commitment to the mission of Swedish Hospital, Part of NorthShore, and adheres to all responsibilities and standards of the organization.

Plans, directs, and provides nutritional care to patients.
Provides initial and follow-up diabetes education sessions, as appropriate.
Conducts diabetes and/or nutrition education classes when needed.
Conducts Indirect Calorimetry measurements and interprets results.
Electronically documents all aspects of nutrition care.
Assists with inpatient diabetes education and coordination of care.
Tracks patient and program-related intervention and outcomes data.
Communicates recommendations, including planned follow-up, with referring providers.
Participates in conferences and meetings as appropriate; represents the work unit on organizational initiatives.
Attends special meetings and rounds as assigned.
Assists with quality improvement audits and projects as assigned.
Performs other duties as requested by Nutrition Director.

PATIENT CARE/AGE SPECIFIC RESPONSIBILITIES AND QUALIFICATIONS
Independently anticipates and always responds appropriately to the age related patients' psychosocial and physical needs. Independently obtains and integrates clinical information in terms of age and developmental needs of patients and responds appropriately.

Sits at desk 50-75% of day; On feet and walking 25-50%
