POSITION TITLE: Program Manager (SCaN Program)
LOCATION/DEPT: Red Shield Corps
REPORTS TO: Community Services Director
FLSA CATEGORY: Exempt

OUTCOMES

Build a trusting and supportive relationship with youth 14 to 24 years old who are at the highest risk of violence in Englewood and West Englewood community, and support youth in meeting their needs and long-term goals. Program Manager will also develop, cultivate, and maintain community partnerships and referral processes with Chicago Department of Family and Support Services (DFSS)-awarded delegate agencies, other service providers, violence reduction organizations, and City of Chicago Departments.
ESSENTIAL DUTIES AND RESPONSIBILITIES.
Outreach and engagement
Receive referrals, assign Navigators to a referral, and assist Navigators in recruiting youth through community outreach and engagement with referral partners (e.g., law enforcement, schools, street outreach, community organizations, etc.)
Monitor and support Navigators as they are conducting outreach, enrolling youth, building a trusting and supportive relationship with youth, and supporting youth in meeting their needs and long-term goals.
Service planning and coordination with youth
Leverage a robust network of service providers within the targeted community areas to meet youth's individualized needs.
Develop, cultivate, and maintain community partnerships and referral processes with DFSS-awarded delegate agencies, other service providers, violence reduction organizations, and City of Chicago Departments.
Training and professional development
Delegate agencies will have structures in place to assist Program Manager in providing reflective supervision and supports necessary for Navigators to be successful (e.g., initial, and ongoing training and coaching, case consultations, self-care plans, etc.)
Identify and provide ongoing skills training necessary for Navigators to be successful in their role.
Data entry and review
Monitor youth service and goal plans and transition plans
Review data weekly in Cityspan to monitor caseloads, ensure accurate data entry, assess program quality and identify gaps in services.
Review data in CitySpan during Active Contract Management meetings with DFSS and determine next steps to implement best practices and address program challenges based on meeting discussions.
Provide clear and consistent opportunities for youth feedback and engagement regarding programming, community referral partners, and family engagement.
Education and training
Participate in Active Contract Management meetings with DFSS and DFSS-sponsored professional development trainings.
Participate in CitySpan trainings
Other duties
As assigned by the Community Center Director.
REPORTING RELATIONSHIPS
This position reports to the Community Services Director
This position supervises the Navigators (Caseworkers).
In contacts related to this position's duties, this individual acts as a representative of The Salvation Army and its mission.

PERFORMANCE MEASUREMENTS
This individual will be evaluated on how effectively the outcomes of this position are achieved, as well as the timeliness, accuracy and completeness of accomplishing assigned goals.
This individual should be able to follow all SCaN program model policies and procedures to ensure compliance with Chicago Department of Family and Support Services (DFSS) and contract and required standards.

A 30, 60, and 90-day performance review will be conducted.
EDUCATION/EXPERIENCE:
Bachelor's degree in social work, Non-Profit Management, Education, or related field required; master's degree Preferred.
Licensure preferred but not required (LPC/LCPC or LMSW/LCSW).
At least three years of experience in case management or program management.
Federal Fingerprint Background check required.
Current Cardiopulmonary resuscitation (CPR) and First Aid Certification required.
Illinois Mandated Reporter Training required Certificate, & Acknowledgment of Mandated Reporter Status Form (annual)
Safe from Harm required.
Caseworker Certification Program (CCP) within 90 days of employment.
