Position Highlights:
Position: Social Worker
Department: Behavioral Health
Location: Arlington Heights, IL
Hours: 8am-4:30pm
Shift: Days / Every other weekend or every Saturday or every Sunday
Part time/Full Time: Per diem
What you will need:
Master's degree from an accredited school of Social Work required.
Current LSW (Licensed Social Worker), LPC (Licensed Professional Counselor) or Clinical Psychologist to practice in the State of Illinois required. LCSW (Licensed Clinical Social Worker), LCPC (Licensed Clinical Professional Counselor) preferred. AMFT or LMFT plus 2 years departmental experience accepted.
Minimum of 2 years' experience in a behavioral health environment.
Current CPR certification.
Completion of training and certification in CPI.
General Summary:
Provides crisis stabilization, case coordination and discharge planning services to patients and families/significant others who are in need of continued behavioral health care following discharge. In collaboration with other members of the interdisciplinary healthcare team, assesses immediate and ongoing needs of the patient, provides acute behavioral interventions and offers recommendations for continued care options. Prepares and updates medical records including plan of care. Determines collateral supports, nature of discharge needs, available resources, patient and family response to recommendations and arranges final discharge plan. Directs patients to designated residential or supportive living options, community based placement/ treatment or other resources as necessary.
