Overview

Works under the program rules and guidelines of the Illinois Department on Aging (IDOA) Community Care Program (CCP) as the Care Coordination Unit (CCU) for Boone & Winnebago Counties. Conducts comprehensive needs assessments in the participant's home or health care facility. Determines eligibility for CCP. Develops a care plan to meet the participants' and caregivers' needs and assists in gaining access to and receiving needed services. Must be able to work and communicate effectively with participants who have cognitive and/or physical impairments as well as people from a variety of social, economic and cultural backgrounds. Must be able to travel to/from the office and participant homes/facilities. Ability to work with a wide variety of agencies within the community to meet needs.
Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES
Performs comprehensive assessments in the participant's home or health care facility to assess need for community services. Determines eligibility for CCP, accurately administers and scores Mini Mental Status Exam (MMSE) and Determination of Need (DON) and collects needed financial information.
Creates a care plan to meet the needs of the participant and caregiver. Demonstrates ability to authorize CCP services and coordinate referrals to other benefits/services available to the participant.
Completes Choices for Care assessments for Nursing Home or Supportive Living Facility placement
Demonstrates knowledge of the various types of assessments, billing codes and documentation required.
Accurately completes Medicaid applications when indicated.
Advocates, intervenes and follows-up in a timely manner to ensure services remain in place and are appropriate to the changing needs of the participant. Documents and completes follow-up on critical events. Updates care plan and completes redeterminations as needed.
Meets established timelines for assessments and critical event follow-up.
Rotates weekend on-call duties as required by IDOA for weekend assessments.
Completes paperwork timely and accurately. Responds to participant/family inquires and phone calls in a timely manner.
Participates in departmental and all-agency staff meetings; offers constructive input to the group.
Completes annual educational requirements for IDOA and Mercyhealth.

EDUCATION & EXPERIENCE
Case Manager minimum qualifications shall be a minimum of 1 year experience in needs assessment and care planning, and:
RN or a BSN, or have a BA/BS degree in social science, social work or related field
One year of program experience (defined as assessment, provision, and/or authorization of formal services), may replace one year of college education up to and including four years of experience replacing a baccalaureate degree; or
LPN with one year of program experience which is defined as assessment of and provision of formal services and/or authorizing service provision.

CERTIFICATION/LICENSURE
Approved CPR or BLS certification required within 90 days
Successfully complete IL Department on Aging Care Coordinator Certification
Maintain a valid driver's license and reliable transportation with proof of vehicle insurance when requested.
Have and maintain an acceptable driving record per Mercy's HR-01 Fleet Safety policy.
