Job Title: Hospice Social Worker
Details: 13-week assignment with the potential to extend. Schedule is 5x8 days Monday-Friday.
Minimum Requirements:
- Active IL Social Worker license
- Hospice experience and HCHB experience preferred
- Competitive pay and benefits
- 401k eligibility after 6 months of employment
