Job Description

Heritage has full-time counseling, assessment clinician, and case management positions that work with individuals with substance use and co-occurring disorders. Services provided include individual, group, and family therapy. In addition, services are provided both via in-person and telehealth sessions.
Positions include both community-based and onsite positions and community-based positions. Services will be provided in various settings, including individual homes, community settings, community agencies, healthcare providers, and Heritage.
Positions available include
Counselor: Outpatient Substance Use ($5,000 sign on bonus available)
Counselor: Medication Assisted Recovery ($5,000 sign on bonus available)
Case Manager: Outpatient Substance Use
Assessing Clinician: Residential Rehabilitation and Crisis Residential
Knowledge, Skills and Abilities:
Ability to engage individuals when they are asking for help and decrease barriers to accessing treatment;
Ability to learn and practice evidence-based treatment services including medication assisted recovery;
Knowledge of community resources including psychiatric, substance use, medical, and/or other community services;
Ability to meet people where they are at in their recovery, be non-judgmental, and ability to build rapport quickly;
Ability to navigate electronic health records.
Core Responsibilities:
Face-to-Face and video assessment, individual counseling, and group counseling services to adults;
Telephone, video, and face-to-face contact with clients participating in substance use treatment;
Provides case management to refer and link individuals to community resources;
Provides engagement to clients who may need additional support to re-engage in treatment;
Completes written assessments, treatment plans, and progress notes for all assigned clients;
Telephone and face-to-face engagement of clients who may have missed individual or group treatment, a doctor's appointment, or scheduled medications;
Coordination and active participation in the referral and linkage to all healthcare including primary care, specialty care, testing and treatment for high risk conditions, mental health, dental, and vision;
Assistance with developing positive supportive activities (re-establishing relationships with friends and family members, support groups, wellness activities, or identifying/participating in positive social activities that support recovery);
Participate in a team-based approach to providing treatment including collaboration with other clinicians, nursing, healthcare providers, probation, DCFS, and other partners invested in the individual's recovery;
Attend clinical staffing(s) to collaborate with providers to coordinate treatment including counselors, engagement staff, nursing, and clinical leaders.
Education and/or Licensure Requirements:
Master's Degree or Bachelor's degree in a human services field who has five years' experience working with individuals with substance use, mental health, or dual disorders, and has shown dedication and passion in providing treatment to individuals with substance use disorders;
Required to obtain an LPC, LSW, or certification as a Certified Alcohol and Drug Counselor (CADC) within 2 years of employment;
Minimum of 21 years of age;
A valid driver's license, reliable means of transportation, and proof of automobile insurance coverage are required.

Access to Federal Student Loan Forgiveness - Heritage is a National Health Service Corp Site, which provides federal student loan forgiveness for eligible licensed counselors, nurses, social workers, and substance use professionals approved through the program.
