Overview

Expected pay for this position is $42.53 - $50.04/hour. Actual pay will be determined by experience, skills and internal equity.
POSITION SUMMARY:
Reporting to the Regional Manager of Care Management is responsible for effectively providing daily leadership for Care Management functions at assigned Medical Center. Responsible for assessment, developing and implementing the care processes for care coordination, transition of care and discharge planning in collaboration with frontline staff, team leads, facility leaders, and Regional Care Management leadership. Responsible for ensuring the services and programs provided by the staff achieve established process and outcome measures. Responsible for consistent application and follow through with established policy and procedure around Case/Care Management functions as outlined by Ministry Care Management. Utilizes leadership skills and competencies to create and enhance a culture that supports the Care Management. In accordance with strategic initiatives, incorporates effective change management philosophies in all Performance Improvement efforts.
Qualifications

REQUIRED QUALIFICATIONS:
Approximately 3 to 5 years progressively more responsible leadership related work experience necessary in order to gain comprehensive knowledge and expertise needed to develop a strategic perspective and independently solve complex process problems in a collaborative manner in a rapidly changing healthcare environment.
Knowledge and understanding of nursing theory and practice.
Knowledge of healthcare business principles and practices, and an in-depth knowledge of performance improvement and change management methodologies including statistical and financial analysis at a level normally acquired through completion of a Master's degree or obtain a Master's degree in Nursing within 5 years.
Interpersonal skills necessary in order to communicate effectively with physicians, Medical Center staff, and regulatory personnel, and lead work of subordinates.
Analytical skill necessary to develop, implement and administer policies, procedures.
Care Management certification for adult or pediatric as appropriate within 3 years of hire.
