Job Description

The Client Transition Funds Manager (multiple positions open) at Lutheran Social Services of Illinois in Des Plaines, IL will carry a caseload of clients to provide care management services, including virtual check-ins, needs assessments, linkages to needed community resources. This position does not have the authority to hire, fire, or make personnel decisions for employees. Engage with clients to determine needs and priority items for independent living. Complete authorization requests, including clinical summaries to advocate for client access to services. Monitor client spending during the transition process from nursing facilities into the community. Manage internal and external Transition and Flex Funds requests. Ensure Pre and Post Transition Case Managers use funds following program guidelines. Reconcile funds for all clients. Gather and maintain records of requests, reconciliations, and all funds-related reports. Provide training to Case Managers on Transition and Flex funds, how to use funds properly, and management protocols. Manage prior authorizations with Health Insurance Providers for all mental health adult case management programs. Telecommuting available.  Salary: $37,814.00 - $61,386.00 / year

Minimum Requirements: Master’s degree in Social Work, Counseling, or related field, plus 1 year in a clinical occupation conducting assessments and therapy services with adults. Must include 1 year of experience with each of the following: (1) counseling clients or patients regarding personal issues, with a focus on mental health, substance use, or trauma history; (2) maintaining client records in CareLogic, Remote Patient Monitoring, and Caspio; (3) obtaining authorization for various mental health services, including Community Support Team, Community Support Individual and Case Management from client’s Managed Care Organization (insurance company) in order to avoid a delay of services or disruption in treatment; (4) completing skills and needs assessments with clients in order to facilitate appropriate community linkages and supports; and (5) crisis intervention and de-escalation.
