Job Description

Reporting to the Clinical Director(s) of assigned healthcare team, serves as a specialty area expert and consultant to the staff, patients, families and operational team who work in various clinical settings. Functions as an expert clinician, role model, consultant, researcher, and educator for patients, families, and staff. Acts as a change agent and case management consultant in the health care delivery system to improve the quality of care across the continuum. Utilizes the nursing process in the delivery of patient care including assessment, planning, implementation and evaluation of patient outcomes. Revises plans of care as indicated. This job description was developed in accordance with the Illinois Nurse Practice Act and the ANA Scope and Standards of Practice.

REPRESENTATIVE FUNCTIONS:

Utilizes specialty expertise to provide consultation services to health care providers and consumers. Consults with the healthcare team on complex patient needs. Facilitates and coaches staff in assessing, defining interventions and problem solving on complex patient care needs. Provides direction and support to patient, family, and health care providers in addressing ethical and legal concerns. Coordinates development, implementation, and evaluation of standards of practice in specialty area with the multidisciplinary team.

Responds to learning needs of health care team, and community groups. Designs, implements, conducts and evaluates comprehensive educational programs relevant to specific patient populations in collaboration with staff and leadership.

Serves as expert clinical resource for the nursing staff and health care providers. Collaborates with nursing team to direct patient care, and to achieve optimal patient outcomes. Demonstrates advanced cognitive and psychomotor capabilities in the diagnosis, treatment, and evaluation of human responses to actual and potential life threatening health problems. Individualizes critical pathways for complex cases collaboratively with nursing team.

Assists with the process of case management by developing and updating critical pathways and analyzing variances from expected plan of care. Develops strategies to minimize or resolve variances as requested. Identifies expected outcomes that incorporate cost and clinical effectiveness, patient and satisfaction.

Demonstrates expertise in performing designated privileges per collaborative agreement and as defined by job responsibilities.

Collaborates with leadership team to evaluate patient and financial outcomes, and redesign processes to optimize quality outcomes. Explores strategies to reduce length of stay and resource consumption within specific patient populations. Works collaboratively with physicians and team members using cost effective clinical outcomes to compliment practice guidelines.

Promotes evidenced based findings into specialty nursing practice and standards. Conducts, participates in, and disseminates clinical research. Integrates relevant research outcomes into clinical practice and teaching content.

Adheres to all Northwest Community Hospital standards, policies, and procedures.

SPECIAL SKILLS AND ABILITIES REQUIRED:

The critical thinking skills necessary to assess and meet the educational needs of staff and evaluate staff's performance and to develop methods of enhancing the quality of nursing care.

The interpersonal skills necessary to relate effectively with various levels of hospital personnel.

The written verbal and presentation skills necessary to provide educational programs.

The organizational skills necessary to balance duties as assigned.

KNOWLEDGE, PRACTICAL EXPERIENCE AND LICENSURE/REGISTRATION REQUIRED:

The completion of a Master's Degree in Nursing.

Certification in assigned specialty area.

Three years' nursing experience in assigned specialty area.

Licensure to practice Nursing in the state of Illinois and Advanced Practice Nursing license.
