Position: Mental Health Clinical Therapist II
Location: LOMG Mokena
Full Time
Hours: Monday-Friday

A Brief Overview:
Providing quality clinical therapeutic services to patient populations and their families/significant others in a physician office or other settings. Serving as a member of the interdisciplinary team, supporting the organization's treatment program and philosophy and assuring the delivery of quality treatment. Orienting and mentoring new staff to the program.

What you will do:
Demonstrates the knowledge and skills necessary to provide care appropriate to the age of assigned patient population. This includes knowledge of the physical and psychological needs of the patients as well as the ability to respond appropriately to those needs.
Conducts psychosocial assessments by interviewing the patient and their significant others to obtain history and identify preliminary issues for treatment focus.
Contacts appropriate referral sources and/or previous or current tertiary care providers to inform of client's enrollment in treatment, obtain relevant collateral clinical data and coordinate care.
Develops, implements and monitors progress of the patient's individualized treatment plan.
Provides individual, couples and family process-oriented counseling sessions as appropriate using various professional treatment modalities.
Participates in regular clinical team meetings and provides input regarding case discussions on the psychosocial, disease management and case management needs of patients.
Demonstrates an understanding of, and adherence to, all procedures and legislation which apply to patients cared for in a Mental Health and/or Substance Abuse treatment setting.
Informs attending physicians (psychiatrist, primary care physician or midlevel provider) in the incidence of psychotropic medication management, of client's treatment progress and/or significant changes in emotional/mental status, which undermine client's daily functioning.
Petitions clients and effectively transitions them to inpatient care in the event of the need for psychiatric hospitalization.
Demonstrates a comprehensive knowledge of community resources (12 step groups, support services, educational resources, etc.) to assure appropriate supplemental care or follow up services after termination.
Demonstrates a comprehensive knowledge of community resources (inpatient treatment settings, recovery homes and residential treatment facilities) and medical allied staff expertise to assure appropriate transition to elevated levels of care for clients requiring more intensive treatment services.
Maintains accurate and timely medical records including psycho-social assessments, family sessions and daily progress notes. Provides descriptions of therapeutic interventions and patient response for discharge planning process.
Participates in activities that enhance professional growth and development.
Serves as a representative for LOH by speaking about the program at community events.
Demonstrates a willingness to participate in committees to improve clinical care.
Demonstrates conduct in keeping with LOH ethical standards.
Complies with LOMG safety, infection control and security program.

What you will need:
Masters Degree Required
2 Years of experience in a behavioral health setting
Knowledge of developmental psychology, disease pathology and clinical needs of assigned population
Ability to maintain information which is highly confidential
Ability to clearly summarize pertinent clinical information via written correspondence and medical records documentation
Ability to demonstrate self-control in volatile or hostile situations
Ability to set therapeutic limits and maintain therapeutic distance with clients; maintains therapeutic boundaries with patients during treatment and following termination
Licensed Clinical Professional Counselor (LCPC) - Illinois Department of Financial and Professional Regulation (IDFPR) Required Or
Licensed Clinical Social Worker (LCSW) - Illinois Department of Financial and Professional Regulation (IDFPR) Required Or
Licensed Marriage and Family Therapist - Illinois Department of Financial and Professional Regulation (IDFPR) Required And
CPI Certification - Crisis Prevention Institute (CPI) Required And
AHA Heartsaver CPR - American Heart Association (AHA)
