Description
The ClinicalTeam Lead, RNreflects the mission, vision, and values of NM, adheres to the organization's Code of Personal Conduct and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
The Clinical Team Lead, RN provides support to the clinical staff by facilitating efficient patient flow, assisting with various patient care activities, and performing related administrative functions. This role leads the work of Staff Nurses and Certified Medical Assistants, ensures efficient patient flow through the clinic, maintains neat, clean and safe patient care areas, promotes patient satisfaction by interacting with patients in a professional, friendly manner, and safeguarding patient confidentiality.
Under the direction of the Practice Administrator or Clinical Practice Admin/Nurse Manager, this position regularly assigns, instructs and verifies the work of other nurses and medical assistants, provides training and orientation of new employees, and identifies and helps resolve staff and clinical operations issues.
Responsibilities:
Leads the work of clinical staff within the practice.
Serves as a role model and resource to clinical staff.
Provides training and orienting to all staff nurses and medical assistants, and maintains training and orientation materials and records.
Creates and manages the staffing schedule.
Monitors and evaluates policies and procedures to suggest operational changes.
Participates in conferences and meetings, and represents the work unit on organizational initiatives
Ensures efficient patient flow through the site.
Performs all aspects of patient care by utilizing evidenced based practice in a manner which optimizes patient safety and reduces the likelihood of medical/health care errors.
Follows the six medication rights and reduces the potential for medication errors, administering medications based on the age and clinical condition of the patient.
States rationale for clinical decisions based on critical thinking.
Assesses, analyzes, validates and integrates data to assess patients' physical psychosocial, rehab, safety, learning, spiritual, cultural, age-specific, special risk and self-care needs. Identifies patient care needs, problems and variances. Identifies patient care needs, problems and variances and incorporates into care plan.
Identifies the patient's learning needs, abilities, preferences and readiness; also, identifies support systems, coping mechanisms and barriers to learning.
Explains procedures, therapies and treatments to patient and family, evaluates and documents patient and family response to teaching. Uses appropriate resources to provide education.
Involves patient/family in the formation of the plan of care. Develops, implements and updates an individualized plan of care based on identified patient care needs. Includes nursing, teaching, continuum of care and discharge planning.
