Description:

The Community Outreach Intervention Projects seeks a Full-Time Social Work Aide II, Medical Case Manager to work at their storefront offices. The Medical Case Manager will provide direct services under AIDS Foundation Chicago's Co-operative Case Management system to community-based clients living with HIV/AIDS. The case manager will carry a caseload of 25-60 clients with high acuity ratings based on multiple diagnoses involving substance use, mental health, HIV status, and physical problems predating HIV. The case manager will provide client-centered, confidential services that link clients with a core, including legal assistance, medical transportation, emergency financial assistance, food, and housing. The aim is to help individuals living with HIV access primary medical care and medications, identify and remove barriers to medical care, and ensure adherence to the prescribed treatment plan to achieve viral suppression.

Duties & Responsibilities:

Determine client eligibility and enroll clients in case management. Develop initial service plans and link clients to appropriate social and medical services. Submit a reassessment as required and update service plans; revise plans when significant shifts occur in clients' needs.
Assist clients with private/public health insurance through effective benefits management practices. Maintain and use reminders in Provide and Outlook to track needed follow-up for client ADAP and other application submissions.
Receive and appropriately document telephone and face-to-face meetings with program participants as required to assess needs. Work with medical care providers (e.g., case conferencing, care conference) to ensure comprehensive medical care is provided and monitored based on the client's medical and psychosocial needs.
Maintain confidential client records, scanning and uploading documents using agency guidelines, including processing transferred records.
Performs chart audits to ensure compliance with program requirements and promote successful outcomes during funding agencies' site visits. Participates in monthly case conferencing with the UI Health Community Clinic Network (UCCN) medical team.
Receive, record, and transmit messages from program participants, communicate with community partners, and respond to case management consults and referral requests to address client needs.
