Description
This position serves in an expanded nursing role working in collaboration with the division Family Medicine providers that provide care to prenatal and postpartum patients. This position will coordinate care and provide support to the providers to ensure a care model that is conducive to the delivery of quality, efficient, and cost-effective healthcare services. Primary focus is on women's health, pregnancy, postpartum care and care of newborn. Assist with the transitions from the hospital to home and follow-op of recommended post-delivery care for both mom and newborn. This position involves a high level of interaction, coordination and education.
 Examples of Duties
Direct Patient Care 80%
1) Works under the direction of the Faculty providers at Center for Family Medicine to provide nursing support to assist with the comprehensive care of OB patients; including scheduling, care coordination, support in clinic and patient education.
2) May attend monthly meetings with faculty members at CFM that provide care to OB patients to gain understanding of and participate in the patient's care and treatment. Assists team in proactively identifying care needs or linking with applicable services to facilitate quality of care.
3) Within the health care team, identifies patients at higher risk for complications and assures coordinated efforts are made to break down barriers and implements a patient-centered, nursing plan of care that promote health
improvements 4) Provides telephone triage for incoming calls from patients, pharmacies and other health care agencies regarding care of OB patients.
5) Assist providers with scheduling and patient education during discharge to ensure there is appropriate transition of both mom and newborn to their outpatient care team.
6) Places follow-up calls to patients within 3 days of delivery to confirm status, provide necessary education and identify any immediate concerns and problem solve any treatment gaps identified
7) Conducts additional follow-up calls as needed during the care to ensure that treatment plans are followed.
8) Support patient needs and identify barriers, provide support and education, connect w/ services necessary to follow recommended plans of care.
9) Assists with rooming patients, vital signs, reconcile medication, assist providers as necessary during the visit and provide necessary patient education at the end of the clinic visit. Actively involved in patient education needs surrounding their care.
11) Follow-up on overdue orders to ensure that appropriate testing is completed and maintain quality care delivered.
12) Work directly with the care team to ensure quality of care delivered.
13) Provide education to staff and Resident providers focused on care of pregnant patients, women's health, well child care and pediatric vaccinations.
14) Serve as a Pediatric Vaccine Lead in the clinic and provide support to providers and staff as needed
15) Serve as clinic lead for school physical clinics by providing support and education to staff and providers.
16) Coordinate follow-up care for children birth - age 5 years to avoid gaps in care.
11) Maintains accurate, up to date documentation for all case management activities.

Administrative/Teaching 20%
1) Provides annual and monthly orientation for residents and students regarding OB, women's health services. Assist with annual education surrounding school physical and pediatric well child care.
2) Assists in collection and analysis of data for quality improvement projects.
3) Keep accurate records of Resident deliveries needed for Residency Graduate Medical Education submission.
4) Participates on internal quality improvement teams
5) Participates on quality improvement committees in the Center as well as externally through the school and at the hospital, as needed.
6) Works with the health care team to continuously evaluate processes, identify problems and propose improvement strategies to enhance deliver of care model.
 Qualifications

1)  Licensed as a registered professional nurse in the State of Illinois (If an Illinois Resident is licensed as a professional nurse in a state other than Illinois, the applicant must meet the criteria established by the Illinois Department of Financial and Professional Regulation to obtain the proper licensure within five (5) months of the date of appointment.); AND

2) Bachelor's degree; AND

3) 2 years of public health or specialized nursing experience

Condition of Employment:  Pursuant to the State Universities Civil Service System, an out-of-state resident who is hired into this position must establish Illinois residency within 180 calendar days of their start date.
