$3000-$4800 Sign On BONUS.  Bonus amount with a 2 year signed commitment.

Position Summary
Occupational Therapists are healthcare professionals who evaluate and treat individuals of all ages who have medical problems or health related conditions that limit their abilities to move and perform functional activities in their daily life. The Occupational Therapist establishes a plan of care based on evaluation of patient. The Occupational Therapist follows their treatment plan of care to improve patient’s ability to perform activities of their daily life, documents progress of treatment and modifies specific treatments in accordance with patient status, and within scope of treatment plan of care and physician’s protocol.
Duties and Responsibilities
Reviews medical chart
Performs Occupational therapy evaluation to determine occupational therapy diagnosis and plan of care
Provide short-term care to hospital patients who are recovering from an accident, surgery, trauma, or illness
Communicates occupational therapy plan of care with occupational therapy assistant when appropriate
Performs treatment to patient based on plan of care to improve patients ability to perform functional activities in their daily life including therapeutic exercise, manual therapy, neuromuscular reeducation, therapeutic activities, fabrication of custom splints/braces, modalities, and other treatments as indicated based on treatment plan of care
Instructs patients on home exercise programs, basic living skills and care, and use of adaptive equipment when necessary
Communicates with other occupational therapists, occupational therapy assistants, physician, rehabilitation team, and other health care interdisciplinary team members regarding patient progress and plans of care
Completes initial evaluations, progress notes and discharges summaries at scheduled intervals within 48 hours of date of service to justify medical necessity and skilled nature of treatment
Completes daily treatment notes within 24 hours of date of service to justify medical necessity and skilled nature of treatment
Accurately completes encounter form with charges for daily treatment that matches documentation on day that treatment is performed
Instructs, motivates, safeguards, and assists patients as they complete exercise and other functional activities
Observe patients during treatments to compile and evaluate data on patient’s response and progress and modifies treatment appropriately
Maintains patient confidentiality
Supervises Rehabilitation technicians
Supervises Occupational Therapy Assistant
Assists in training and orientation of new staff
Schedule patients for appointments
Print home exercise programs utilizing Exercise Pro, HEP2Go, VHI, or other exercise program
Access patient medical records for rehabilitation prescription, MRI/Radiology reports, surgical reports, and physician notes
Contact physicians to request new prescription or medical records for patient care
Change pillow cases to maintain clean treatment area for patients when needed
Wipe down treatment table when needed
Other duties as assigned
POSITION QUALIFICATIONS
Education
Graduation from a nationally accredited program.
Illinois licensure.
CPR/AED certification.
