Work Schedule: 8 Hr (8:00:00 AM – 4:30:00 PM)

Summary:

The scope of practice of the Clinical Pharmacist Specialist covers direct and indirect patient care, education, medication safety, performance improvement, and consultation. The Clinical Pharmacist Specialist is responsible for the provision of both operational and clinical pharmacy services to a general population of patients. Exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.

Required Job Qualifications:

- Bachelor of Science in Pharmacy.

- Illinois Pharmacist License.

- Minimum of four years of Clinical Pharmacy experience OR PGY 1 residency completed within the last seven years, which includes Clinical / Specialty Pharmacy experience.

- Knowledge in intravenous admixture services, intravenous drug therapy and chemotherapy.

- Must be able to work effectively with others to provide safe and effective drug therapy.

- Excellent problem solving skills and the ability to work independently and quickly in responding to and resolving a wide range of problems.

Preferred Job Qualifications:

- BLS Certification.

- ACLS Certification (or shall be obtained upon hire).

- Board of Pharmaceutical Specialties Board Certification.

- Doctor of Pharmacy degree and residency certificate.

- Experience and strong knowledge base in specialty drug therapies.

Responsibilities:

1. Participate in the provision of pharmaceutical care through chronic disease state management receiving referrals from RUSH providers

2. Provide therapeutic recommendations within approved scope of pharmacy protocols and provide thorough and precise education counseling

3. Have excellent customer service and communication skills to be able to work with outpatients and team members in a professional and calm manner

4. Aid in transitions of care whenever possible, facilitating transitions from inpatient to outpatient with regards to pharmacy management

5. Serve as a liaison to other healthcare providers

6. Assists with the strategic implementation of Department of Pharmacy initiatives

7. Participate in Pharmacy Department operations per departmental standards 8. Serve as a preceptor for pharmacy students and postgraduate year 1 and 2 (PGY1 and PGY2) residents

9. Coordinate medication ordering, preparation, dispensing, and administration when applicable for target patients or medications

10. Participate in the RUSH and Department of Pharmacy committees as assigned/relevant to practice area

11. Supports value-based care population health initiatives under Rush Health

12. Implement and adhere to all drug use policy and formulary decisions of the PNT committee

13. Participate in safety event (SE) reporting

14. Complete medication use evaluations and new drug or therapeutic class reviews for PNT as assigned

15. Maintain working knowledge of medical center and departmental policies and procedures

16. Participate in quality improvement projects, per interests and/or assignment by the supervisor

17. Be skilled with appropriate technology associated with the clinic (Epic, Standing Stone software, point of care coagulation technology)

18. Participate in appropriate billing documentation for the patient visits

This is a grant-funded position through 2025 with the ability to work remotely and some on-site responsibilities.
