Position Overview
The Division of Developmental Disabilities is seeking to hire a Licensed Practical Nurse II for the Choate Mental Health and Developmental Center located in Anna, Illinois to perform a variety of practical nursing functions. Provides direct nursing care based on an individual’s nursing plan of care. Assists with the admission, transfer, and discharge of individual. Administers medication; and assists in the maintenance of clinical records.
Job Responsibilities
- Performs a variety of practical nursing functions.
- Observes and records individuals’ symptoms, injuries, and illnesses.
- Implements interventions, including steps in progressive limit setting, e.g., verbal, redirection, restraints.
- Provides nursing input to interdisciplinary team.
- Attends mandatory training and in-service training/workshops to facilitate the development and refinement of therapeutic skills.
- Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.
Minimum Qualifications
- Requires Illinois license as a practical nurse.
- Requires one year of practical nursing experience.
*This class is included as an Upward Mobility Program credential title.
Conditions of Employment
- Requires ability to physically restrain individuals as necessary to prevent injury to individual or others.
- Requires ability to work after business hours, weekends, and holidays.
- Requires ability to utilize office equipment, including personal computers.
- Requires the ability to travel in the performance of job duties.
- Requires ability to pass the IDHS background check.
- Requires ability to pass a drug screen for drugs prohibited from recreational use under Illinois Law.
